Objectives: This study is to evaluate diagnostic effects of the cytology and fluorescence in situ hybridization(FISH) for the detection of urinary bladder epithelioma , then expanding new diagnosis method for bladder cancer; To compare the rates of chromosomes aberrations in paraffin block cells between recurrent bladder cancer tissues and non-recurrent tissues, then concluding the value of chromosomes-based FISH in predicting recurrence of urinary bladder epithelioma; Meanwhile, applying FISH assays in the fusion gene detection for the prostatic carcinoma tissues and hyperplasia ones, so as to explore aided method on improving PCA puncture-positive rates without false-positive rates.Methods: A mixture of 15 urine samples, final diagnosis of which was bladder urinary epithelioma, and 35 non-epithelioma samples were collected, including 15 cases with hematuria. Diagnosis was based on cystoscopy in all patients. Then adding in fluorescent labelled probes to detect chromosomes 9p16,3,7 and 17 chromosomes aberration , recording positive or negative, taking a urinary exfoliocytology examination simultaneously ; Besides, 32 pathological paraffin block samples collected , the patients of which had been performed surgery therapy from Jan. 2007 to Mar. 2008 in our hospital, were continuely divided into two groups: recurrence group(18 cases) and non- recurrence group(14 cases), finally applying FISH assays to detect chromosomes 9p16, 3,7 and 17 chromosomes aberration.;Additionally , 10 cases of PCA tissues and hyperplasia tissues from Jan. 2009 to Dec.2009 were selected, both of which were aimed at detecting fusion genes TMPRSS2-ERG,TMPRSS2-ETV1,TMPRSS2- ETV4 using three types FISH probes.Results: The sensitivity of FISH based on chromosomes 9p16, 7,3,and 17 for the diagnosis of the bladder urinary epithelioma is 86.7% , which is significantly higher than cytology , the specificity is 88.6%. However,there is no significant difference between FISH assays and cytology detection, with no influence despite of hematuria;Additionally, the rates of chromosomes 9p16 and 17 aberration in the recurrent cancer tissues are obviously higher than non- recrudescence, without chromosomes 7 and 17 possessing statistical significance The sensitivity of the fusion genes TMPRSS2-ERG,TMPRSS2-ETV1,TMPRSS2- ETV4 detection is 62%, the specificity is 100%.Conclusion: The sensitivity of FISH for the detection of bladder cancer is superior to cytology, but there is no difference in the specificity between two assays;the rates of chromosomes 9p16 and 17 aberration in the recurrent cancer cells are significantly higher than that of non-recurrence, thus it can be showed that the rates of recurrence are higher than normal ones respectively;The FISH assays in the fusion genes detection is much likely to improve the detection rate of PCA, with no increasing in false-positive rate.
