Objectives:All patients with coronary artery disease of the subject were choised to be the on-pump coronary artery bypass grafting, target vessel through the right coronary bypass graft blood flow measurements, to observeâ‘ Relationship between right coronary artery with degree of stenosis and average blood flow and pulsafility index of saphenous vein.â‘¡By monitoring the the average flow, diastolic graft flow waveform, pulsatility index (PI) value of right coronary artery bypass grafting, to understand the situation of graft patency in clinical surgery, improve the success rate. Methods;Coronary angiography stenosis than 50%of coronary heart disease patients receiving coronary artery bypass surgery with cardiopulmonary bypass, using LIMA (LIMA) to left anterior descending artery, all the other targeted vessels were used the great saphenous vein (SV).Using the transit-time flowmeter (TTFM) to measure and record the blood flow,the PI values and the waveforms.All patients based on the right coronary artery stenosis were divided into the A, B, C three groups, to observe the relationship between the right coronary artery with degree of stenosis and average blood flow and pulsafility index of saphenous vein. Results:All patients in the CCABG surgery were used the SV to anastomosis to the RCA, the anastomotic patency rate were 100%. All symptoms of angina pectoris were disappeared after the surgery. The results of blood flow were:The average flow rate of C group is greater than group A and group B (P<0.01); The average flow rate of group B is greater than the group A, the difference was statistically significant (P <0.01);The PI of group C is less than group A and group B, the difference was statistically significant (P<0.01); The PI of group B is less than group A, the difference was significant (P<0.05); Mean arterial blood pressure between the groups were no significant difference (P> 0.05). Spearman rank correlation analysis showed that the mean flow, PI and the right coronary artery stenosis was correlated (rs=0.624, rs=-0.452, p<0.001). Conclusion:The right coronary artery bridge of the SV is a double perfusion, mainly to diastolic perfusion, and the internal mammary artery to the main different diastolic perfusion. The PI values is the most reliable indicator of patency to reflect the blood flow, but we cannot determine whether there are anastomotic problems solely based on that the PI is greater than or equal to 5. The higher the RCA stenosis is, the greater the blood flow of SV Bridge and the smaller pulsatility index (PI) value will be in the CCABG. This study shows us that there has a high positive correlation between the degree of stenosis and the mean flow.
