Integration Of Traditional Chinese And Western Medical Clinical Path Way Applying For The Operation Of Lumbar Disc Herniation Patient

Object:To explore the way for the integration of traditional Chinese and Western medical clinical path way applying for the operation of lumbar disc herniation patients, which meets the actual situation in China,analysize all kind of variation during implementation.It will be demonstrated than it is a practical and significal way in enhancing the medical quality, specifying the process of diagnosis and treatment,shortening the hospital days,lowering the medical cost, advancing the reasonably utilization of medical resources, providing experience to clinical pathway.Method:1,Work out the table of integrative medical clinical path way.2, The control group is the patient from January 1st,2008 to December 31,2008 in Traditional Chinese medical hospital attached Luzhou Medical College.The experimental group.is the patient from January 1st,2009 to December 31,2009 in Traditional Chinese medicine hospital attached Luzhou Medical College. 3,The general descriptive statistics,T-test and rank testing methods are used to compare the effect of two groups.For example:the total number of days hospitalized, preoperative hospital days, postoperative hospital days, hospital costs, the rate of traditional Chinese medical treatment, clinical effect, the degree of patient satisfaction.4.Analysising the variance during clinical pathway.Result:After applying the clinical pathway, the total number of days hospitalized, preoperative, postoperative hospital days are shorten, hospital costs decreased, the rate of traditional Chinese medical treatment are increased, the score of patient satisfaction is increased.1,Compare with the traditional group, the clinical pathway group of hospitalization days, preoperative hospitalization days and postoperative hospitalization days are reduced, mainly to the hospitalization days, postoperative hospitalization days.The results shows that implementation of clinical pathways in patients with lumbar disc herniation, the average hospitalization days was 10.86±2.08, compared with a decrease than 3.33 days of traditional group, two groups were statistically significant (P<0.001);the clinical pathway group of preoperative days in hospital are decreased by 0.59 days,two groups were statistically significant (P=0.026);The clinical pathway group of postoperative hospital stay is decreased of 2.78 days compared with traditional group, and statistically significant compared to the traditional group (P<0.001).2, For hospitalization costs, medical costs are reduced, while the Chinese medical costs are increased. The total cost of traditional group is 8059.21±786.7 yuan, while the the clinical pathway group is 5817.69±223.40 yuan, lowering than the traditional group of 2241.52 yuan,there is significantly difference between the two groups by T test (P <0.001);The cost of traditional Chinese medicine is 337.53±63.56 yuan rose to 391.48±66.88 yuan; there is a significantly difference between the two groups by T test (P<0.001);3,For clinical effect, the traditional group is 68% excellent, the clinical pathway group is 79%, clinical pathway is better than the traditional group. There is a significantly difference between the two groups by ranking test (P= 0.036);4,For patient satisfaction,the clinical pathway group is (97.56±3.29),higher than the traditional group(95.53±4.11),there is a significantly difference between the two groups by T test (P =0.005);5.For the rate of Chinese medical treatment the traditional group is 83%,while the clinical pathway group is 96%,there is significantly difference between the two groups by ranking test(P=0.022).6.The Variance from Clinical Pathway:34 mutation occurred during implementation of clinical pathway,7 patients out of clinical path way.Conclusion:1.the total number of days hospitalized, preoperative, postoperative hospital days are deceresed after applying the clinical pathway.2.After applying the clinical pathway, the hospital costs,clinical effect, patient satisfaction are deceresed, while the rate of traditional Chinese medical treatment are ieceresed.3.Clinical pathway improved clinical efficacy and patient satisfaction; 4.The clinical pathway guaranteed the quality of medical and nursing care and saved health care resources.5,It is feasibility for the wide usage of clinical pathways in China.
