Clinical Study On Relationship Between Hepatic Depression Syndrome Of Type 2 Diabetes Mellitus And HbA1c, Chronic Complications

Objective:The aim of this study was to explore the relationship between Hepatic Depression Syndrome of type 2 diabetes mellitus and HbAlc, and provide a instruction for blood glucose control by Chinese medicine of smoothing the liver.Objects and Methods:According to the WHO(1999) diagnosis standard,65 type 2 diabetes mellitus patients were chosed as research objects.Then according to the macroscopic diagnostic criteria for hepatic deprssion syndrome established by Chen Jiaxu,Chen Qinghong,Xu Hongyan, et al,65 type 2 diabetes mellitus patients were divided into hepatic deprssion syndrome group and non-hepatic deprssion syndrome group, including 31 males,34 females age from 42-64 years old. The average age was 52.69±4.950 years old. After the patients were brought into the research team, according to 1997 "Quantizating diagnose of syndrome" of Xu Dihua and Xu Jianqiu, a card was designed to value the hepatic deprssion of 65 cases. HemoglobinAlc(HbAlc),IMT,TCSS, proteinuria was detected,and the morbidity of diabetic complications was recorded in every case. The results of them were analyzed in two groups'patients. The HbAlc level and IMT were compared with the scores of hepatic deprssion.Results:1. the average HbAlc of hepatic depression syndrome group was (9.509±1.960)%, and non-hepatic depression syndrome group (7.980±0.834)%. The HbAlc level of hepatic depression syndrome group was higher than that of non-hepatic depression syndrome group, P<0.05.2.The morbidity of coronary heart disease and ischemic cerebrovascular disease of hepatic depression syndrome group was higher than those of non-hepatic depression syndrome group, P<0.05,and the morbidity of diabetic foot, diabetic nephropathy, retinopathy, peripheral neuropathy of hepatic depression syndrome group was higher than those of non-hepatic depression syndrome group,but difference was not statistically significant,P>0.05.3.Carotid artery intima-media thickness (IMT) of hepatic depression syndrome group was 1.03±0.13, and non-hepatic depression syndrome group 0.94±0.15, IMT of the hepatic depression syndrome group was thicker than that of non-hepatic depression (P<0.05).The difference with statistical significance.4.In the hepatic depression syndrome group, the Toronto clinical neuropathy score 0-5pm(no lesions) 19 cases, score 6-8pm(mild disease) 9 cases, score 9-12pm(moderate disease) 6 cases, score≥12pm(severe disease) 1 case,and in non-hepatic depression syndrome group, the Toronto clinical neuropathy score 0-5pm(no lesions) 20 cases, score 6-8pm(mild disease) 6 cases, score 9-12pm(moderate disease) 4 cases, score≥12pm(severe disease)0 case, but difference was not statistically significant,P>0.05.5.In the hepatic depression syndrome group, urinary protein negative19 cases, trace9 cases, 1+:5cases,2+:3cases,3+:2cases;and in the non-hepatic depression,the urinary protein negative:20 cases, trace:2 cases,1+:5 cases,2+:2 cases,3+:1 case,but difference was not statistically significant, P> 0.05.6.There was positive correlation between the scores of hepatic depression and the HbA1c in the hepatic depression syndrome group, r=0.773,P<0.05;there was positive correlation between the scores of hepatic depression and IMT in the hepatic depression syndrome group, r=0.558,P<0.05.Conclusion:The HbA1c level of hepatic depression syndrome group was higher than that of non-hepatic depression syndrome group,P<0.05,and there was positive correlation between the scores of hepatic depression and HbA1c level. The morbidity of diabetic chronic complications of hepatic depression syndrome group was higher than that of non-hepatic depression syndrome group.It implies that hepatic depression plays an important role in blood glucose control in the type 2 diabetes mellitus patients.When one patient is diagnosed hepatic deprssion syndrome,using the method of smoothing the liver is helpful to control the blood glucose level and prevent the complications of the diabetes mellitus,and increase his or her living quality.
