| Objective:Observed with the TCM symptom features of Intestinal Endotoximia of Chronic Liver Failure(CLF) in sixty patients, and explore the its possible mechanism. To investigate the clinical effect of colon dialysis retention enema with Chishao Chengqi Decoction in the treatment of Intestinal Endotoximia of Chronic Liver Failure(CLF).Methods:Sixty patients were selected from Shenzhen Third People's Hospital inpatient liver disease patients from March 2008 to March 2009, which average age was 38.27±9.86years. All of them diagnosed as Chronic Liver Failure (CLF) intestinal blood endotoxin disease. Sixty patients were randomized by Clinical Evaluation of Traditional Chinese Academy of Sciences with the clinical research center of Central Randomization System (Network). There are 12 cases of incipent,13 cases of medium,5 cases of terminal stage in treatment group; compared with 13 cases of incipent,11 cases in medium,6 cases of terminal stage in control group. Before the treatment,biochemical tests were statistically no significant difference (P>0.05). Treatment group were treated with Chishao Chengqi Decoction with colon dialysis retention enema based on western medicine. Chinese herbal enema Chengqi Decoction (including Radix 30g,Magnolia officinalis 15g,Zhishi 15g,rhubarb 6g,etc.), 1-2 times per week for 4 weeks as a course. Statistics 60 patients demographic data, predisposing factors,time of infection and main clinical features. By two or more deputy chief physician of Chinese medicine practitioners under the tongue and pulse and other dialectical, two groups were statistical treatment of symptoms and signs before and after integration. During the statistical treatment of 1 week and 1,2 months of blood,including total white bloodcell(WBC),hemoglobin(HGB),plateletcount(PLT),the proport ion of neutrophils(N),red blood cell count(RBC).1 week during treatment on biochemical indices,including totalbilirubin(TBiL), albumin(ALB),alanineaminotransferase(ALT),aspartateaminotransfe rase (AST),prothrombin-time(PT-SEC),acetylcholinesterase(CHE). Du ring the first month of treatment 1,2endotoxin(ET). Treatment efficiency of two groups,mortality and complication rates and length of stay in both groups.Result:(1) 60 cases of major clinical symptoms to fatigue 60(100.0%), fever 60 cases (100.0%), dark urine in 55 cases (91.6 %), anorexia in 52 cases (86.7%), abdominal distension (51 cases 85.0%) of the highest incidence.Syndromes of blood stasis in order to account for most of the heat syndrome (48.3%), followed by heat into the Camp Blood (21.7%) and endoretention of damp heat (18.3%). Symptoms and signs points in the fourth week after treatment compared with control was significant difference (P<0.05). (2) Endotoxin treatment group at week 4 decreased to 0.101±0.027 Eu/ml,compared with the control group significant difference(P <0.05). After one month treatment group, elevated levels of ACHE was 7466.71±2164.2U/L, compared with the control group was signif icant difference (P<0.05); red blood cell count (3.59±0.47)×1012g /L,compared with the control group was significant difference (P <0.05); treatment of the first in February, the total bilirubin fell to 76.55±25.22 umol/L, the prothrombin time was 14.70±6.11S, as compared with the control group, significant differences (P<0.05). (3) Two groups in survival rate and hospitalization time were significant differences (P<0.05).Inclusion:On clinical observation of 60 patients showed intestinal endo toxemia in symptom divided by fatigue,fever,yellow urine, anorexia, etc, the performance characteristics of its pathog enesis to the "stasis", "hot" as the main syndrome features. For the pathogenesis of syndrome features,we used retention-enema colon dialysis treatment,the results show that this method in improving the clinical symptom score,lower serum endotoxin levels,reduce mortality and shorten the hospitalization time has a certain effect.
