Objective: To investigate the clinical efficacy of extensive hepatectomy and tentative treatment for surgical in patents with alveolar echinococcosis in advanced stage. Method: A retrospective clinical analysis was made in 41 cases in our hospital from 2000 to December 2010 of AE in whom the lesions were involved in bilateral hepatic vessels and bile duct admitted. They were divided into two groups (extensive resection group and palliative group) .and a comparison was performed between two groups. Result: extensive resection and tentative resection group showed no statistical significance in median in-patient time. Radical resection group took more operative time and caused more hemorrhage during operation. But there's a remarkable decreasing in post operative complications and retaining time of drainage tube compare with palliative group. Conclusion: Even in the advanced stage of Hepatic alveolar echinococcosis case, if there's no heart ,lung , Liver and kidney problems normal coagulation function and if the affected vascular and billiary tract can achieve reconstruction condition after plastic surgery, a radical resection should be performed.
