Background:Cardiovascular disease is the leading cause of mortality for people in China. In order to demonstrate the value of cardiovascular risk factors,this study is to explore the relationship between cardiac risk factors of coronary artery disease and characteristics of coronary artery lesion , and the relationship among Beta2-microglobulin(β2-ΜG),high sensitive C-reactive Protein(hs-CRP), von Willebrand factor(vWF), Seru uA(SUA)which may be potential risk factors for coronary artery disease, and severity of coronary artery lesion. The study may be helpful for prevention and treatment of coronary heart disease.Objective:1. To investigate the relationship between some of potential cardiac risk factors of coronary artery disease (CAD), such asβ2-ΜG, hs-CRP, vWF, SUA, etc.2. By comparing the differences and correlation ofβ2-MG, hs-CRP, vWF, and SUA in the AMI group, SAP group and UAP group to study the value of diagnosising coronary heart disease and the ACS.3. To exploreβ2-ΜG, hs-CRP, vWF, SUA with traditional factors using Syntax score to evaluate severity of coronary artery disease, it may predict coronary events of CAD patients.Methods: From March of 2008 to August of 2009, a total of 288 consecutive patients with chest pain who underwent Coronary angiography were enrolled in the study. Including 96 patients with UAP, 39 patients with AMI, 63 patients with SAP and 90 controls (patients without coronary heart disease). The levels ofβ2-ΜG, hs-CRP, vWF were measured using the ELISA method. The traditional cardiac risk factors such as age,hypertension,diabetes,smoking were recorded. Blood sugar, total cholesterol, triglyceride, HDL-c, LDL-c, Lp(a) were tested. All patients (with or without coronary heart disease) were performed coronary angiography. Coronary stenosis>50% was defined as in coronary artery disease. No stenosis or stenosis <50% in control group. The severity of coronary stenosis was calculated using Syntax score.Results:①The levels ofβ2-ΜG,hs-CRP,vWF in CAD group were significantly higher than those of non-CAD group (P < 0.01). But the uric acid level was non-significantly different between the two groups.②Multiple regression analysis showed thatβ2-ΜG, hs-CRP, vWF may be independent risk factors for coronary artery disease.③The levels ofβ2-ΜG, hs-CRP and vWF in AMI group was much higher than UAP group or SAP group, and the difference was statistically significant (P<0.001, P<0.001, P<0.001). Meanwhile, the levels ofβ2-MG, hs-CRP and vWF levels were significantly higher in UAP group than SAP group.④Correlation analysis demonstrated that,β2-ΜG, hs-CRP, vWF and SYNTAX score was low positive correlated, while LVEF and coronary artery stenosis showed a low negative correlationship. Linear regression analysis showed, the impact ofβ2-ΜG, LVEF, CHOL and age on SYNTAX score was statistically significant, the extent of its impact on the SYNTAX score wasβ2-MG>LVEF>CHOL. Regression equation y=21.23+5.12Xβ2-MG -0.35XLVEF +1.93X CHOL.Conclusion:①β2-ΜG, hs-CRP, vWF may be some independent risk factors for coronary artery disease.②β2-ΜG, hs-CRP, vWF levels were significantly higher in the AMI group, which may predict acute coronary syndrome.③The level ofβ2-ΜG was correlated with severity of coronary artery disease.β2-ΜG,hs-CRP and vWF may be potential markers for ACS.④Examination ofβ2-ΜG level of coronary artery disease patients combinating hs-CRP and vWF may help to understand the extent of CAD and predict coronary events.
