| Objective:The purpose of the study is the preliminary study of influencing factors about postoperative cognitive dysfunction in elderly patients, and then psychologically intervene elderly patients with the treatment of abdominal surgery, and analyse the influence of the occurrence about postoperative cognitive dysfunction in elderly patients with the psychological intervention and treatment of abdominal surgery.Methods:1,Choice one hundred cases of elder than sixty years of age patients under general anesthesia and scheduled for selective non-cardiac surgery from Shanxi Province People's Hospital, assess the preoperative and postoperative cognitive function with neuropsychological testing scale, and analyse the influencing factors of the occurrence about postoperative cognitive dysfunction in elderly patients;2,Choice one hundred cases of elder than sixty years of age patients under general anesthesia and scheduled for selective abdominal surgery from Shanxi Province People's Hospital, and they will be randomly divided into two groups: trial group and control group.Trial group fifty ones, will be given psychological intervention. Control group is fifty ones;3,Statistical Analysis:Spss11.5 statistical software will be applied to record the data. Logistic stepwise regression analysis will be applied to analyse the influencing factorsResult:1,The relevance was significant between the pre-operative hypertension, old age and preoperative depression with the occurrence about the postoperative cognitive dysfunction(P<0.05);2,Postoperative cognitive dysfunction occurred in 15 patients (30%) in control group and 5 patients (10%) in trial group. The incidence of postoperative cognitive dysfunction was significantly reduced in trial group compared with in control group (P < 0. 05 ). To compare with the two groups, determine the concept, orientation and structure of the indicators in the neuropsychological testing have significant improvement and are obviously change (P < 0. 05 ). To compare with the same group in the surgery of before and after, the indicators in the neuropsychological testing of trial group after intervention are not obviously change. The control group without intervention obviously decreased in the mental arithmetic, the concept of judge, orientation, attention, far memory, recent memory, structure and thinking than the surgery of before(P < 0. 05 ). Conclusion:Hypertension, old age and preoperative depression are the influencing factors associated with the occurrence about the postoperative cognitive dysfunction in elderly patients. Psychological intervention in perioperative can decrease the incidence postoperative of cognitive dysfunction in elderly patients about the abdominal surgery. Then the cognitive function of elderly patients in all aspects can improve, especially can obviously improve in the concept of judge, orientation and structure. |
