Backgroud:Urinary tract infection is a common type of infection in the elderly, the symptoms and complications impact on the quality of life for the elderly seriously, and even threaten their lives. Following the aged tendency of population, the health management of elderly draws more attention and the study of urinary tract infection in elderly is more and more urgent.Modern medicine studies for urinary tract infection are mainly focused on the pathogenic bacteria and use of antibiotic,so do their treatments.The traditional Chinese medicine(TMC) plays a conspicuous role in treatment of urinary tract infection,an a result,.treatment combining traditional Chinese medicine and western medicine has been approved by more and more people.In recent years, researches of combined treatment of traditional Chinese medicine and western medicine continue to emerge, but for the pattern syndrome of urinary tract infection, the doctors still hold different opinions.Treatment based on syndrome differentiation is a basic characteristic of traditional Chinese medicine, therefore, the diagnosis and classification of the syndrome has a positive significance for the treatment of the disease.Objectives and Methods:Based on the retrospective study of 84 cases of elderly patients with urinary tract infections from nephropathy department,we analyzed the characteristics of traditional Chinese medicine concluding common symptoms,tongue conditions and pulse conditions, summarized the characteristics of the TMC pattern syndrome.On the other hand, the laboratory indexes were discussed in groups to study the effect of the TCM Syndromes.Purples of this study is to provide part of the basis for the TCM syndromes classification of urinary tract infection and to contribute to the diagnosis and treatment.Results:1.In 84 cases,9 cases are male(10.71%),75 cases are female(89.29%); the average age was (73.84+8.41) years old, and patients among 70 to 79 years old were the most.There is no significant difference in the proportion of men and women in each age group.2.The most common courses of the disease is within 7 days and between 10 days to 1 years, while patients whose course of disease is more than 6 months account for 54.76%.3.Most common symptoms are frequent urination, urgency and odynuria,and the rate of others such as poor sleep, low back pain, fatigue or soreness of waist are more than 40%.4.Dark red tongue (36.90%) and the red tongue (23.81%) are the most common tongue nature;for the tongue moss, thick fur, rot moss and greasy moss account for 40.48%;dry moss accounts 25.00%, no moss; less moss and peeling fur account 13.10%; yellow fur accounts 35.71%,while white fur accounts 52.38%.5.Pulse conditions show that string-like pulse and slippery pulse both have the most proportion over 60%, followed by thready pulse, deep pulse and rapid pulse.Cases of weak pulse,irregular pulse, feeble pulse and floating pulse are few.6.1n syndrome classification, cases mingled with excess and deficiency take 57.14%, while excess syndrome only accounts 29.76%, deficiency syndrome only accounts 13.10%.The heat syndrome is most common in excess syndromes. The kidney qi and spleen deficiency syndrome are the most common deficiency syndromes, while kidney yang deficiency has the lowest frequency.7.There is no significant effect on the blood leukocyte count, the percentage of blood neutrophil, serum creatinine level and urinary protein.8.The patients with spleen deficiency have lower hemoglobin content.9.There is a high level of platelet count in patients with damp.10.Patients with deficiency of kidney yin are more likely to appear more urinary white blood cells than those without the syndrome.11.Patients with kidney qi deficiency usually have a normal level of urine red blood cells and they are less likely to appear more urine red blood cells compared with patients without kidney qi deficiency.Conclusion:TCM syndromes of senile urinary tract infection patients are mainly in intermingled excess and deficiency,while ecxess syndromes are more common than deficiency syndromes.The heat syndrome is the most in excess syndromes. The kidney qi and spleen deficiency syndromes are the most common in deficiency syndromes, while kidney yang deficiency has the lowest frequency.TCM syndromes have some influence on the blood hemoglobin content, platelet count, and microscopic examinations of urine white blood cells and urine red cells.
