ObjectiveTo explore how the severity of periventricular-intraventricular hemorrhage(PIVH)impact on physique and neurodevelopment in premature infants.MethodA total of 89 preterm infants with PIVH who admitted to NICU from Dec.2013 to Dec.2015 were eligible. According to the papile classification, the premature infants were divided into four groups.Group A was gradeⅠ PIVH. Group B was gradeⅡ PIVH.Group C was gradeⅢ PIVH and Group D was gradeⅣ PIVH. The infants with gradeⅠPIVH and gradeⅡ PIVH belong to the low-grade PIVH group. The infants with gradeⅢPIVH and grade Ⅳ PIVH belong to the severe PIVH group. Their clinical data were collected. All of them follow-up in pediatric outpatient of Qingdao women and children hospital after discharge. The differences in physical and neurophysical development of premature infants among four groups were compared. Finding the affects that effected premature infants mental development index(MDI) and physical development index(PDI) factors through multiple linear regression analysis.Results1.The difference of physical development in four groups at different corrected age in infancy.(1)The growth retardation were assessed by body weight. The incidence of growth retardation of four groups decreased before 3 months’ corrected age. And the preterm infants of four groups catch-up growth rapidly before 3 months’ corrected age.(2) There was no statistically significant differences in the incidence of growth retardation, overweight and emaciation among four groups at different corrected age(all P>0.05).(3) There were no significant differences at physical growth indicators such as head circumference, body weight and body length among four groups at corrected age 1month, 2 months, 3 months, 6 months and 12 months(all P>0.05).2. The comparison of neurodevelopment in premature infants with PIVH of four groups.(1) The MDI and PDI scores of group A were significantly higher than group C and D at 6 months’ corrected age(P<0.05). The PDI scores of group B were significantly higher than group C and D at 6 months’ corrected age(P<0.05).(2) The MDI and PDI scores of group A and B were significantly higher than that of group C and D at 12 months’ corrected age(P<0.05).(3) The severe PIVH group had the higher rates of cerebral palsy and developmental delay than the low-grade PIVH group(P<0.05).3.Finding the affects that effected premature infants intelligence development index(MDI) and physical development index(PDI) factors through multiple linear regression analysis.(1)The MDI scores for premature infants at 12 months’ corrected age was correlated with gestational age and the severity of PIVH.The results as follows: regression coefficient B were 1.642,-12.136(P<0.05).(2)The PDI scores for premature at 12 months’ corrected age was related to the rate of PVL, The regression coefficient B were 1.980(P=0.020).Conclusions1. The severity of periventricular-intraventricular hemorrhage has no impact on physical development in premature infants. The preterm infants with PIVH catch-up growth rapidly before 3 months’ corrected age.2. Grade Ⅲ-Ⅳ PIVH can have negative effect on the neurodevelopmental outcomes in premature infants. The infants with the severe PIVH had higher rates of cerebral palsy than infants with low-grade PIVH. The regular follow-up and early intervention in preterm infants with PIVH should be implemented to promote the quality of their lives.3. The severity of periventricular-intraventricular hemorrhage can have negative impact on motor and mental development in premature infants. And the PIVH infants with periventricular leukomalacia can affected the motor development in premarure infants.ObjectiveTo explore how the severity of periventricular leukomalacia( PVL) impact on physical development and neurodevelopment in premature infants.MethodA total of 65 preterm infants with PVL who admitted to NICU of Qingdao women and children’s hospital from Dec.2013 to Dec.2015 were eligible. According to the de Vries classification, the infants with grade Ⅰ-Ⅱ PVL were group A and the infants with grade Ⅲ-ⅣPVL were group B. At the same time, 37 preterm infants without PVL were group C. All of them were followed up regularly at neonatal outpatient department after discharge and complete the intelligence test at 6 months’ corrected age and 12 months’ corrected age. Differences in their clinical data, body weight, body length, rate of growth retardation, mental development index(MDI), psychomotor development index(PDI) and neurophysical development were compared. Finding the affects that effected premature infants intelligence development index(MDI), development index(PDI) factors through multiple linear regression analysis.Result1. The comparison of physical development among three groups at different corrected age.There were no significant differences at physical growth indicators among three groups at corrected age 1 month, 2 months, 3 months, 6 months and 12 months(P>0.05). There were no statistically significant difference in the incidence of growth retardation among three groups(P>0.05).2. The comparison of neurodevelopment among three groups at corrected age of 6months and 12 months.(1) The MDI sores of group B were significantly lower than that of group A and C at6 months’ corrected age(P<0.05).(2) The MDI and PDI scores of group A and group B were significantly lower than that of group C at 12 months’ corrected age(P<0.05). The MDI and PDI scores of group B were significantly lower than group A at 12 months’ corrected age(P<0.05).(3) The incidence of cerebral palsy and developmental delay of group B were significantly higher than that of group C(P<0.05). The incidence of motor retardation of group B was higher than that of group A(P<0.05). The incidence of motor retardation of group A was higher than that of group C(P<0.05).3. Finding the affects that effected premature infants intelligence development index(MDI) and physical development index(PDI) factors through multiple linear regression analysis.(1) The MDI scores of premature infants whose corrected age was 12 months was related to the severity of PVL. The results as follows: regression coefficient B were-8.129(P<0.05).(2)The PDI scores of premature infants at corrected age 12 months was related to the severity of PVL, regression coefficient B were-13.674(P<0.05).Conclusion1. There was no significantly difference in physical development between premature infants with PVL and premature infants whose cranial ultrasound was normal.2. The premature infants with PVL were prone to motor development delay in infancy. The incidence of cerebral palsy was high in premature infants with grade Ⅲ-ⅣPVL.3.The severity of periventricular leukomalacia can have negative impact on the motor and intelligence development of premature infant.
