Study Of New Tpye Of An Ultrasound Contrast Agent In Quantitative Diagnosis Of Hepatic Fibrosis And Cirrhosis

Objection: To observe Hepatic Artery Arriving Time(HAAT),Hepatic Vein Arriving Time(HVAT) and Hepatic Artery To Vein Transit Time(HA-VTT) among liver cirrhotic group,liver fibrotic group and normal group by real-time gray scale contrast enhanced ultrasonograghy(CEUS) and investigate the clinic value of the HAAT,HVAT and HA-VTT for quantitative diagnosis of liver fibrosis and cirrhosis with contrast-enhanced ultrasound.Methods: (1) 20 cases of liver fibrosis and 20 cases of liver cirrhosis proven by biopsy enrolled in this study while 10 cases without liver disease history served as controls. The grade of the liver function in liver cirrhosis include 16 cases of Child-pugh A and 4 cases of Child-pugh B. (2)Use Two-dimensional Ultrasound(2D) to observe the liver and measure the type of Doppler spectrum of the right hepatic vein. Then CEUS was performed with 1.6ml SonoVue bolus injection to determine HVAT and HAAT, the two subtraction is HA-VTT. Comparison of differerences of HAAT,HVAT and HA-VTT results among each group. After analyzed the results by statistical method ,the value (P<0.05) was considered significance. Results:(1) HA-VTT had significant difference among fibrotic group, cirrhotic group and normal group (P<0.05); HAAT had no significant difference among the three groups (P>0.05); HVAT had significant difference beteewn normal group and fibrotic group ,normal group and cirrhotic group (P<0.05). There is no significant difference beteewn fibrotic group and cirrohotic group (P>0.05). (2) HAAT,HVAT,HA-VTT had no significant difference between Child-pugh A and Child-pugh B. (3)The extraordinary wave rate of the right hepatic vein in liver cirrhosis group was more then liver fibrosis group ,and the differenc had statistics significance (P<0.05).Conclusion: Determination of HV-ATT by the CEUS can reveal hemodynamic changes in liver cirrhosis and considered to be a new diagnositic method in quantitative diagonosis of hepatic fibrosis and cirrhosis.
