Objective:To investigate the expression and implication of human CD44 variant in laryngeal carcinoma and adjacent carcinoma tissues and Hep-2 laryngeal carcinoma cells and and investigate the relationship and clinical significance of CD44 variant in laryngeal carcinoma.To evaluate the human CD44 variant tumor metastasis in laryngeal carcinoma diagnosis.Methods:Specimens from 38 cases of pathologic stageⅠtoⅣlaryngeal carcinoma and adjacent carcinoma tissues,each specimen of sample contains one peace of tisue taken from the center of the tumor and adjacent carcinoma tissues taken from larynx away from the tumor edge above 0.5cm.Hep-2 laryngeal carcinoma cells in were inoculated,and regarded as the control group.They were performed to detect the expression of cell adhesion protein CD44 variants (CD44v) in laryngeal cancer and adjacent carcinoma tissues and Hep-2 laryngeal carcinoma cells by using reverse transcription- polymerase chain reaction(RT-PCR).Specific primers at the middle of constitution exon 5 and constitution exon 16 of CD44 were designed . cDNA of laryngeal carcinoma and adjacent carcinoma tissues and Hep-2 laryngeal carcinoma cells were analyzed by RT- PCR. Products of RT- PCR were purified,sequenced and further analyzed by bioinformatics.Results:The results turned out to be as follows:The CD44 sequence possessed 125bp without a completed open reading frame. It was predicted to be standard form CD44,CD44s.A high level of CD44s expression was more frequently found in laryngeal carcinoma(89.5%)and Hep-2 laryngeal carcinoma cells(83.3%)than in precarcinoma lesions tissues(26.3%)(P<0.05). CD44s expression correlated well with histological grades,the lower grade the higher of the CD44s expression(P<0.05). The expression of CD44s in laryngeal carcinoma with metastatic lymph node was significantly higher than that in laryngeal carcinoma without metastatic lymph node (P<0.05). A high level of CD44s expression was found in laryngeal carcinoma with smoking than that in laryngeal carcinoma without smoking. TheⅢandⅣstages laryngeal carcinoma had stronger CD44s positive than those of theⅠandⅡstages (P<0.05). The relative levels of CD44s mRNA in the laryngeal carcinoma and Hep-2 laryngeal carcinoma cells are higher than in the precarcinoma lesions tissues(0.5366±0.2347 vs 0.2340±0.1615,P<0.05;0.5081±0.2184 vs 0.2340±0.1615,P<0.05).The relative level of CD44s mRNA in lymph node metastasis laryngeal carcinoma is higher than that in no lymph node metastasis laryngeal carcinoma(0.5992±0.2286 vs 0.3866±0.1815,P<0.05). The relative level of CD44s mRNA in laryngeal carcinoma with smoking is higher than that in laryngeal carcinoma without smoking(0.5297±0.2241 vs 0.3710±0.1669,P<0.05). And the relative level of CD44s mRNA in tumor of TNMⅢ,Ⅳstaging are higher than that in tumor of 1,2,staging(0.5699±0.2346 vs 0.3145±0.2359,P<0.05). The expression of CD44s mRNA was positively correlated with the metastasis,smoking, clinnical staging and pathological classification but not correlated with gender,location of laryngeal carcinoma(P>0.05).Conclusions:There are over expresion of CD44s which takes part in the genesis and development of tumor at some certain and is one of prognostic factors.Overexpression of CD44s is closely related to metastasis,smoking, clinnical staging and pathological classification. High expression of CD44s may suggest an increased risk for local neck nodes metastasis in laryngeal squamous cell carcinom(LSCC).It indicates that neck dissection and compound therapy should be applied.Detecting CD44s not only can predict the prognosis of patients, but also provide direct information for individualized therapy for tumor.CD44s may play an important role in progression and metastatic potential of laryngeal carcinoma, and may be a biologic marker of laryngeal cancer.
