Objective:To investigate the earlier efficacy and safety of recombinant human brain natriuretic peptide ( rhBNP) on patients with acute heart failure. Methods:A total of 40 patients with acute heart failure were enrolled in the study and were randomizely divided into two groups. 20 patients ( rhBNP group) were assigned to receive rhBNP(1.5ug/kg bolus intravenous injection followed by 0.0075μg·kg-1·min-1 intervenous drop infusion) ,while the other 20 (control group) received Isosorbide Dinitrate or Sodium Nitroprusside. The degree of dyspnea and general condition were evaluated before and at 30min 24h and 72h after drug administration,the volume of fluid intake and urine was recorded 24h after drug administration, the recording of ejection fraction(EF)and left ventricular end-diastolic diameter (LVEDD) were measured respectively in these patients before and after 72h'study. Plasma levels of N-terminal pro-Btype natriuretic peptide (NT-proBNP) were measured respectively in these patients before and after 5—7day'study.The safety of rhBNP was documented by measuring blood pressure, heart rate, electrolyte and renal function before and after treatment, and all the adverse events were registered. Results:After 24.72 hours'treatment.the condition of dyspnea and clinical manifestations were much better in the rhBNP group than the control group (P=0.049,P=0.043), the mean urine volume was significantly greater in rhBNP group than in control group(P=0.01),Plasma levels of NT-proBNP were significantly reduced in rhBNP group compared with control group(P=0.001),while there was no difference in EF and LVEDD between the two groups (P>0.05).Conclusion:The dyspnea and symptoms in patients with acute heart failure can be improved by rhBNP. recombinant human brain natriuretic peptide, rhBNP can reduces plasma levels of NT-proBNP in patients with acute heart failure. bring favorable prognosis to these patients.
