| Fatty liver disease is the accumulation of lipid in the liver or tissue more than 5% or 1/3 per unit area, and now it has climbed to second liver disease after the viral liver disease with a increasing incidence year after year.The ages of the patients are become younger. Medical profession has deep research on the pathogenesis, however,it still has not a clear effective treatment. In China, the majority of fatty liver disease is non-alcoholic fatty liver disease (NAFLD). Its comproud pathogenesis, remains unclear, making it more difficult to effectively treat. Hence, according to the principle of medicine and dialectical theory of governance, we use Self Quzhi Shawei Decoction combined with acupuncture to treat patients with NAFLD and investigate the clinical efficacy. With the rat model of non-alcoholic steatohepatitis as the carrier, we discusses the therapeutic effect and mechanism of acupuncture and traditional Chinese medicine treatment, so as to provide better ideas for mechanism and even treat methods of nonalcoholic fatty liver disease. This article gives a detailed and systematical study and expounds the research progress, clinical and experimental problems of nonalcoholic fatty liver disease from two aspects.The first part is clinical research. Objective:Clinical observation of acupuncture and tranditional Chinese medicine treatment of Self Quzhi Shawei Decoction for patients of fatty liver disease. Methods:Use Self Quzhi Shawei Decoction (giant knotweed rhizome, semen cassiae, and crataegus)to treat 70 cases of fatty liver disease(treatment group), combined with acupuncture (bilateral Zusanli, Taichong, Taixi, Fenglong, Sanyinjiao). According to the clinical symptoms and signs, divide all the treatment group into liver depression qi stagnation, spleen phlegm and damp, and deficiency syndrome patients give additional moxibustion therapy (Zusanli, Sanyinjiao),contrasted to the control group (EssentialeforteN treatment group) 34 cases. Three months is a course.After two courses, observe the clinical symptoms and signs, BMI, liver and spleen CT ratio, blood glucose, liver function and blood lipid changes in indicators before and after treatment. Results:Treatment group can significantly improve the clinical symptoms, improve BMI values, liver function, decreased serum triglyceride (TG), cholesterol (TC) and improve the liver and spleen CT ratio, the total effective rate is 88.57%. The total effective rate of the control group is 67.65%, the effective rate between the two groups are significantly different (P<0.05). the liver and spleen CT value, BMI value, have statistically significant differences after treatment in treatment group and control group (P<0.05). Conclusions:Use acupuncture and tranditional Chinese medicine treatment of Self Quzhi Shawei Decoction for fatty liver disease can achieve better clinical efficacy.The second part is experimental study. Objective:Explore therapy mechanism of acupuncture and traditional Chinese medicine treatment for nonalcoholic.fatty liver disease. Methods:Bulid the rat model of non-alcoholic fatty liver disease,divide them into acupuncture and traditional Chinese medicine treatment group, Chinese traditional medicine treatment group, acupuncture group, western medicine (EssentialeforteN) treatment group.,and give corresponding diet changes.Investigate the change of liver pathology,Liver index,blood lipid,liver function, iron metabolism, insulin and adiponectin,and their connection with oxidatie stress. Results:Acupuncture and traditional Chinese medicine treatment can significant improve liver pathology,deduce the liver steatosis,decrease level of ALT, AST and TG, TC,improve adiponectin and insulin resistance,and, to a certain extent, improve oxidatie stress index.The acupuncture and traditional Chinese medicine treatment group achieve better clinical efficacy than other groups.Conclusions:Therapy mechanism of acupuncture and traditional Chinese medicine treatment for nonalcoholic fatty liver disease may lie in the improvement of lipid metabolism and insulin resistance and oxidatie stress... |
