| Objective:The objective of this trial is to observe the clinical efficacy of the Lianloudanxing decoction and the comparison between the effects on left atrial diameter, platelet p-selectin and glycoprotein exp-ression in surface film before and after patients who have the atria fibrillation causing by phlegm-fire disturbance taking the medicine. The clinical efficacy of this trial was eval-uated from ACC/AHA/ESC Arterial Fiberation Treatment Principles and Guiding Principles for New Drugs Clinical Research of Chinese Medicine to compare the effects and changes of TCM symdromes, TCM symptoms before and after treatment.Methods:From 2009 to 2011, the basic data and incident cases in both Chinese and Western's points were collected from the inpatients who have the atria fibrillation causing by phlegm-fire disturbance in Car- diology Department of Guang'anmen Hospital. Trial 1: Patients were given Lianloudanxing decoction which made into single herbal concentrate granules and administrated one dose ea-ch day divided into twice with warm water for 28 days. According to the difference between Chinese and Western clinical standards, Therapy criteria were divided into significant effect-tive, effective, insignificant effective and aggravated. Observing clinical efficacy, the time of transferring sinus rhythm, symptom integral changes of western curative effect and TCM syn-drome and symptom before and after treatment. Trial 2:The control group patients were giv-en aspirin, oral, 100mg entericcoated metformin hydrochloride, once; the TCM. group patie-nts were given asprin and Lianloudanxing decoction which made into single herbal concent-rate granules and administrated one dose each day divided into twice with warm water for 28 days. We observe the heart left atrial diameter, platelet p-selectin and glycoprotein expression in surface film in partly TCM patients by using flow cytometry to detect platelet p-selectin and glycoprotein expression in surface film GPI-Ib/GPⅢa (PAC-1). The statistics was carri-ed out by the analysis of the gender and age of the patients, underlying disease, medication and the West-ern poin- tsCD62P and PAC-1. Result:Trial 1①25 cases in total 49 canses were transferred sinus rhythm (51.0%). The shortest time of transferring is 3days and the longest is 15days, on average 7.08±3.06 days,14 cases less than 7days and 11cases more than 7days;②the decoct-ation effects:significant 8 cases (16.3%), effective 34 cases (69.4%), invalid 7cases (14.3%), aggravate 0 cases (0%), and total effective percentage 85.7%;③the decoctation effi-cacy on TCM syndromes:significant 10 cases (20.4%), effective 31 cases (63.3%), invalid 8 cases (16.3%), aggravate 0 cases (0%), and total effective percentage 83.7%;④By TCM integral analysis, patients with atrial fibrillation before and after taking the medication, symptom scores decreased significantly (P<0.01);Trial 2:①TCM or control groupspatients with atrial fibrillation before and after taking the medication, the heart left atrial diameter has no significant differences (P> 0.05);②compared the platelet P-selectin before and after treatment, decrease signific-antly (P< 0.01), and TCM is more effetive (P< 0.01);③compared PAC-1 before and after treatment, decrease significantly (P<0.01), and TCM is more effective than control group(P<0.01).CONCLUSION:①most of the people with atrial fibrillation due to the disturbance of phlegm-fire are male patient,frequently occurring at the age of72.08±1.08, the main causes of the disease are coronary heart disease, hyperte- nsion, diabetes mellitus;②Decoction with Lianloudanxing can improve concomita-nt symp- to ms in treating atrial fibrillation with phlegm-fire disturbing the heart③with the treatme- nt of Lianloudanting Decoction, there is significant efficacy on both of the major and minor symptoms of atrial fibrillation due to the phlegm-fire disturbance in heart.④Lianlou danxing Decoction can reduce the CD62p and PAC-1 expression significantly, and reduce the platelet activity, have anti-platelet effect, thrombosis and thromboembolism of the prevention and treatment. |
