ENGLISH ABSTRACTIntro du c tio iiStroke characterized with high incidence rate,high disabling rate,high death rate,high relapse rate and etc.is one of the Three most common causes of deaths in human beings,which cause enormouse mental,material and economic loss of family,society and country. Ischemic stroke (IS) accounts for 80 per cent of all stroke patients. And its incidence is becoming higher and higher in recent years. So how to prevent acute ischemic stroke (AIS) effectively and reduce its incidence rate,disabling rate and death rate,and improve the quality of patients have became a critical task faced the world medical field. But there is no satisfactory therapy for AIS so far. Per nasal therapy is a traditional one in traditional Chinese medicine (TCM). Naoxing Nasal Spray was developed. It is a pure natural herbal medicine that mainly contains the TCM herb Ligustigum Chuanxiong and Acorus Tatarinowii. Naoxing Nasal Spray can activate Qi and improve blood circulation, expel the pathogenic wind to eliminate phlegm, wake up patients from unconsciousness, which can be used for AIS.Part One ReviewThere is an extensive literature dealing with the use of Ligustigum Chuanxiong and Acorus Tatarinowi in pernasal therapy for cerebrovascular diseases. The literature has shown that pernasal therapy is based on both 1CM theories and ~vestern medicine theories. Two of the mostly used herbal medicine in pernasal therapy for AIS are Ligustigum Chuanxiong and Acorus Tatarinowi. Pharmacological studies suggest that Ligustigum Chuanxiong can increase cerebral perfusion, prevent platelet aggregation and thrombosis, scavenge free radicals, block calcium-channel, improve the balance of TXA2/PGI2, inhibit apoptosis and that Acorus Tatarinowi can sedate, hypnotize, treat convulsion, improve memory and hypoxia tolerance, lower blood-lipid, and prevent thrombosis.Part Two Clinical StudyObjective To observe the effect of Naoxing Nasal Spray on AIS and explore its mechanismMethods According to the random,contrast and blind principle,sixty-threepatients with AIS were divided randomly into treated group and contrast group. In treated group, forty-one patients were treated with Naoxing Nasal Spray. In contrast group, twenty-two patients were treated with Nimodipine. The serumal levels of free radicals lipid peroxides (LPO), supercxide dismutase (SOD), GSH-.PX, ET/NO, TXB2/6-K-PGF1 ~, fibrinogen, the maximal aggregation rate of platelet, the dynamic change of hemorheology and the effects of the two groups were evaluated after one course of treatment of 3 weeks.ResultsIn treated group, the metabolic disorder of ETINO and TXB~/6-K-PGF1 ~, the levels of fibrinogen and free radicals (LPO, SOD, GSH-PX), the maximal aggregation rate of platelet and hemorheology were markedly improved (P<0.05 or P<0.0l) after treatment. Compared with control group, the levels of 6-K-PGF1 and GSH-PX , and the maximal aggregation rate of platelet in treated group were significantly improved (P<0.05 or P<0.0 I). The total effective rate and the excellent rate in treated group were 90.2% and 70.3% respectively. And those in contrast group were 90.8% and 68.2% respectively. There were no significant differences in the overall effective rate or excellent rate between treated group and control group (P>0.5).Conclusion Naoxing Nasal Spray could reduce the AIS patients?serumal levels of LPO~ EL TXB2 , fibrinogen and the maximal aggregation rate of platelet, and meanwhile increase the levels of SOD~ GSH-PX.~ NO and the activities of 6-K-PGF1 ~.
