Objective To investigate the blood plasma levels of endothelin and nitrit oxidc in Hypertension and Diabetes and their relationship with insulin resistance. Methods In this study , 100 subjects were divided into three groups , essential hypertension ( n=25 ) NIDDM ( n=36 ) essential hypertension with NIDDM (n=39) . An age and sex matched health population worked as control (n=20)Sex, age height, weight. waistline. hipline blood pressure wcrc recorded. All of them were carried out with OGTT and insulin release examination. And blood greases ET- 1 NOlVwere examined at the same time. Then calculated the IAI, BMI, WHR with formula, analyze the relationship between the change of vasoactive substances and Insulin resistance. Results 1. The mean plasma ET- 1 level in three patient groups were all higher than control, and there were difference between hypertension with or without NIDDM and NJDDM. The change of N03 - was contrast with ET- 1, the level ofNOYin three patient group were all decreased compared with control, but there were no differences between three groups. The linear correlate analysis and step regression analysis showed that ET- 1 is i elated with IAI. 2. FBG, FINS, BMI, WHR, TG, ET-1 are related with IAI. 3. The IAI reflects the degree of insulin resistance. AUC is not suitable in the diabetes. Conclusions The IAI can reflect the degree of Insulin resistance well. FBG. FINS BMI, WHR,. TG,. ET- 1 are related with insulin resistance. The interaction between ET and NO plays an important role in hypertension with or without NIDDM. ET- 1 is an independency risk factor in the occur of insulin resistance.
