| High-site anal fistula is difficult to cure in coloproctological department ,so it is always the focus for coloproctological surgeons to research into . Of all therapies ,thread-drawing in high fistula canal plus open drainage is the most common operation. However,it has many defects, such as wide wound, slow recovery , severe anal pain redundancy of scar tissue and etc, so the therapy seriously damage the anal function and some patients suffer from a series of complications.Fistula-milling operation is a surgical therapy in which the anal fistula is not cut open .Milling cutter used in the self-innovatory milling operations was invented by Wang Caoyuan who is the professor of HuBei college of TCM . The fistula-milling operation is advanced in high-site anal fistula. The experiment was designed to compare the fistula-milling operation with the routine surgical therapy.The objective was to estimate the feasibility, reliability of fistula-milling operation using in high-site anal fistula and probe into the mechanism. MethodsThere were 85 cases totally in the experiment which were divided into tested groups(38 cases) and controlled groups(47 cases) at random. The tested group were treated with the new surgical operation-fistula-milling operation, while the controlled group were given the traditional surgical operation-incision in low fistula canal and thread-drawing in high fistula canal. After operation, to evaluate which was better ,we observed the operating time ,the clinical curative effect ,the recovery course, the postoperative complications, recurrence rate and etc . ResultsThere were no static significance in the clinical curative effect, the recurrence rate between the two groups(p>0.05),but there was notable difference in the recovery course ,the operating time between the two groups(p<0.01),the tested group showed the belter than the controlled group, whafs more, the two groups had statistic significance in anal smooth degree(p<0.05),which showed the surgical method had little effect on the anal function. The two group had notable difference in postoperative anal pain and postoperative complications ( P<0. 05 ) , so the tested group had obvious advantages in avoiding the slow recovery, anal pain,anus stricture,anus incomplete incontinence and other complications. ConclusionsApplication of the fistula-milling operative method reveals that this method is simple in manipulation, reliable and satisfactory in theraeputive results. The new surgical operation has little effect on anal physiological function because of keeping more perianal muscles and skins. So it has great clinical value and social effects . |
