Liu Wei Di Huang Decoction Treatment Of Early Diabetic Nephropathy With Microalbuminuria Clinical Observation

Objective:To observe the curative effect and safety of Modified LiuWei DiHuang Decoction to Diabetic Nephropathy(DN) on the basis of basic therapy, especially the functions of reducing albumin and protein as a whole in the urine of patients, and compare with simple basic therapy. To explore the mechanism and offer the scientific basis for the further improvement and clinic application of Modified LiuWei DiHuang Decoction.Methods:60 cases of DN, who belong to the symptom-complex of asthenia of Kidney-Yin and deficiency of Qi , are randomly divided into treatment group and comparison group. The patients in the treatment group take Modified LiuWei DiHuang Decoction on the basis of basic therapy, while the patients in the comparison group are just treated with basic therapy . After eight weeks, we compare the differences between groups, including the symptoms and relevant clinical parameters, and study the data with statistical analyses.Results:1.The total clinical effective rate of treatment group, 90%, with statistical analysis, is better than that of comparison group.2.The effective rate of treatment group in the symptom-signs, 93.33%, is much better than that of comparison group with statistical analysis.3.Both treatment group and comparison group have showed statistical significance in terms of reduction of uric albumin excretion rate(AER) as well as protein content in 24 hours' urine. Furthermore, the decline in the treatment group is more obvious than that in the comparison group.4.During clinical experiment, there are no serious badness and side-effect inrelation to Chinese herbal medicine.Conclusions:1 Taking Modified LiuWei DiHuang Decoction on the basis of basic therapy can significantly improve the clinical effect of DN, and alleviate the clinical symptoms, such as lumbago, polyuria, dizziness, thirst, lassitude, deficiency of vital energy and so on.2. Taking Modified LiuWei DiHuang Decoction on the basis of basic therapy can obviously decrease AER and protein content in 24 hours' urine of DN. There is noside-effect at the same time.3. The mechanism of Modified LiuWei DiHuang Decoction can be as follows: (1)Enalapril, which falls into the category of ACE1, can partly obstruct Ang I totransform into Ang II .Therefore, it can decrease the manifestation of TGF- β : (2)Modified LiuWei DiHuang Decoction can collaborate with Enalapril to further prevent Ang I from transforming into Ang II ; (3) Modified LiuWei DiHuang Decoction can improve glomerular sclerosis through decreasing hyperlipemia;(4) Modified LiuWei DiHuang Decoction can protect indirectly the Kidney by mediating the balance of immune-neuroendocrine interactions.
