Part I Comparison of Doppler Tissue Imaging,16-Slice Spiral CTand Coronary AngiographyObjective: To evaluate the application and clinical value of Doppler tissue imaging (DTI) and 16-slice spiral CT(16-S CT) in coronary artery disease (CAD) and compare DTI and 16-S CT.Methods: Fifty suspected patients were performed by DTI and 16-S CT. First, Doppler Tissue Velocity (DTV) and Doppler Tissue Pulsed wave (DT-PW) were used to detect the myocardial wall velocity, the data were recorded from six walls and sixteen segments of left ventricular. Second, these patients were performed by 16-S CT. The patency of coronary artery and other branches were examined. The distribution and extent of coronary arterial stenoses were assessed. The findings demonstrated on 16-S CT and DTI were compared with those seen on selective coronary angiography (SCA).Results: (1)The sensitivity; specificity; positive and negative predictive values for the diagnosis of CAD by DTI were 89.5%, 91.67%,94.44%, and 73.33%; and the data by 16-S CT were 92.26%,83.33%,94.87% and 90.89%.(2)The peak systolic velocities (Vs) of segments correlated directly with the extent of coronary artery stenoses (r=0.861,p<0.01) .(3) The accurate ratios in diagnosis of CAD were similar in the whole between the DTI and 16-S CT, and the latter was higher than the former.(4)The parallel experiment of DTI and 16-S CT improved the sensitivity (97.36%), and negative predictive values for the diagnosis in coronary artery disease.Conclusions: (1)16-S CT and DTI as invasive examination are valuable methods in detecting coronary artery disease. (2) Wall motion velocity correlated will with extent of coronary artery stenoses.
