Objectives:To observe the effect of the L-Amlodipine and Irbesartan on thecarotid endarterium and arteria elastic in hypertensive patients.Methods:The 69 patients with primary hypertension were from 2003 to 2004in the First Hospital of ShanXi Medical University. They were divided into three groups bymatched age and sex. The group C (control group, n=24) treated with indapamide(2.5mg,semel in die) and group A (test group, n=26) with L-Amlodipine (2.5mg,semel indie) and group B (test group, n=19) with Irbesartan (150mg,semel in die) . Collectgeneral data that the blood pressure, blood fat and blood sugar from patients beforetreatment. The humeral arterial carotid arterial mean intimal-medial thickness(MIMT) andendothelium-dependent dilation(EDD), non-endotheliun-dependent dilation(NDD) weremeasured three times with resolution ultrasonography for each group. The indices and bloodpressure were measured again after 3 month, 6 month of treatment respectively.Result:①There were no difference in three groups on the blood pressure, blood fatand blood sugar before treatment(P>0.05). The group A, group B and group Care effectively in reduction of blood pressure after treatment.② L-Amlodipine andIrbesartan are effective to improve the MIMT compared the pretherapy withpost-treatment(P<0.0001). Compare group A with group B, there are no differencein the two groups on improvement MIMT (P>0.05). ③Compared with group C, group Ahad a significantly better effect on preventing atherosclerosis after 6-month post-treatment(P<0.05). L-Amlodipine had lower the score of Course(P<0.05). There was no differencein preventing atherosclerosis and lowering the score of Course (P>0.05). Compared withgroup C, group B have difference obviously after 6-month post-treatment(P<0.05). ④Theall three drugs can improve EDD(P < 0.05). The effect were similar(P > 0.05).⑤L-Amlodipine and Irbesartan have no effect to NDD(P>0.05). The three drugshave no difference in improvement of NDD(P>0.05).conclusion: In addition to reduction of blood pressure , L-Amlodipine andIrbesartan have an effect on regression of abnormal function andstructure of large arteries. L-Amlodipine is better than Irbesartan ondelaying the progression of atherosclerosis.
