The Intervention Of The Law Of The Acute Phase Of Stroke Tongfu France

Objective The aim of this study is to explore the traits of the heat-phlegm andsthenic-fu syndrome of apoplexy, and the clinical laws of the application of the method of purging fu-organ in the acute stage of apoplexy.Materials and Methods Through the literature search and clinical investigation,I have found that the heat-phlegm and sthenic-fu syndrome plays a very important role in the acute stage of apoplexy,also the method of purging fu-organ is very effective. So I have reviewed 216 selected patients with the heat-phlegm and sthenic-fu syndrome in the acute stage of apoplexy from DongZhiMen Hospital of Beijing University of TCM and XiYuan Hospital of China Academy of Chinese Medicine Sciences. I have inquired general datas of the patients,special spmptoms of apoplexy,certain spmptoms of TCM,the aid examination results,principles of treatment and prescriptions.I have classified the symptoms which have high appearence by Frequencies,Factor and Classify analysis to explore the inner relation of them.I have gotten the changing rules of the symptoms in the acute stage by statistics. I also have studied the laws of the prescriptions of the two hospitals.Results In this group of patient, 75.80% of the apoplexy patients is over 60 years,and male is more than female.69.91% of the patients have gotten hypertension and 43.94% have suffered from constipation before apoplexy occured.24.07% of the patients fall ill in pneumonia during the course of apoplexy.The formation of the heat-phlegm and sthenic-fu syndrome in the acute stage of apoplexy is related to senile constitution,widely-changing climate and abnormal coordination function of the lung,hypertension due to constipation,and improper food.These pathogenic factors lead to the accumulation of phlegm and stagnant blood,blocked fu-viscera-qi,then apoplexy happens.After the disease occures,the flow of qi becomes abnormal and phlegm-heat congests worse and worse,so the heat-phlegm and sthenic-fu syndrome is formed. So, I conclude that the pathogenic location of apoplexy is in the brain,also it has close relation with the lung and the large intestine. Keeping the stool free can not only help controlling hypertention but also prevent the occurance of apoplexy.Symptoms' Factor and Classify analysis shows that the nature of the pathological mechanism is the deficent root and excessive branch syndrome. The excessive branch includes heat-phlegm,blood stasis,sthenic-fu and the deficient root includes both deficiency of qi and yin. So when the method of purging fu-organ is used in clinical,the methods such as dispelling phlegm,activating blood circulation,nourshing yin,clearing fire,calming liver and extinguishing wind shoud also be used to treat the whole,especially strengthing anti-pathogenic qi to conduct pathogenic factors out of the body and prevent purgative therapy damaging qi and yin.Constipation,red tongue,yellow and greasy tongue coating,taut and slippery pulse are the main symptoms for the heat-phlegm and sthenic-fu syndrome which appears as the first syndrome in the acute stage of apoplexy.As soon as they appear the method of purging fu-organ can be used.This syndrome can also be trasformed from orther syndromes. Symptoms such as foul breath,rough breath which can often be seen before the sthenic-fu and should be taken as the indicatoin of using the method.The proper use of purging fu-organ can prevent the development of the disease.After being treated,this syndrome often changs into the phlegm-stasis obstructing the network syndrome,the qi vacuity and blood stasis syndrome,the qi and yin vacuity syndrome.Through the study of the changing rules of the symptoms and the principles of treatment of DongZhiMen Hospital and XiYuan Hospital,I find that the high incidence stage of the syndrome is in the first week, especially 3~5 days after apoplexy occurred. The prescription used most frequently includes Banxia,Dahuang, Gualou,Danxing,Chenpi,Danshen,Zhishi,Fuling,Yujin,Huangqin,Danggui, Niuxi.When the method of purging fu-organ is used,supporting right and banking up the root is also very important. The right time of using the method of purging fu-organ is the key to the curative effect.The usual dosage of Dahuang in decoction is 9-10g and the largest dosage is 15g.The sign of normal fu-viscera-qi uninhibited is not only bowel freeing but also the tongue coating,foul breath,abdominal fullness vanishing. The disease will be cured with purging fu-organ,elimnating heat,dispelling phlegm. The main aid of purging fu-organ is not only making the stool free but also purging fu-organ to rugulate functional activities of the Zang-viscera and Fu-viscera.Conclusion Through combining theory-study and clinical experiments,I thinkthat the method of purging fu-organ is effective in the prevention of apopley,thus it can be used widely in clinical treatment of apoplexy. We've made better sense of the pathogen and mechanism of the heat-phlegm and sthenic-fu syndrome in the acute stage of apoplexy and enriched the therapy of apoplexy and the clinical rules of the application of the method of purging fu-organ. So the article is of great significance in both thoery and clinical practice.
