Objective:Rheumatoid arthritis is a kind of self-immune disease characterized by joint synovitis. Although modern medical science has made great progress in study of pathogenesis and pathology, the etiology is indefinite. Being lack of perfect medicine and medical scheme in treatment, non-steroidal anti-inflammatory drugs, hormone, immune-resist dose, cytotoxic drug are commonly applied to relieve pain and control patient's plight with the drawbacks of having side reaction and high cost. Chinese traditional medicine curing rheumatoid arthritis has a long history and various treatments, especially effective in anti-inflammation, reliving pain and having an immune effect of resistance and adjustment. Based on the Chinese medicine theory of Bi syndrome and Tujia and Miao medicine theory, Qingjiang Rheumatic medicated wine is composed of select Enshi folk medicine, which can cure rheumatism, accelerate blood circle, eliminate swell, relieve pain, etc. According to long clinical observation that the medicated wine has an effect in relieving pain, especially obvious in curing rheumatoid arthritis. Based on the elemental theory of Chinese traditional medicine, our study is to know the action of Qingjiang Rheumatic medicated wine in curing rheumatoid arthritis and observe its clinical effect through experiments with modern methods and technology. By further exploring the essence of folk medicine and national medicine, we can provide effective and less side-reaction medicine for Rheumatoid arthritis, and offer a scientific theory for the clinical application of Qingjiang Rheumatic medicated wine. Meanwhile, we can promote the development of our national medicine and economy by making full use of the local folk medicine resources.Methods:1. Experimental research: dividing 120 mouse into 3 groups (half of them by gender), which are used to do 3 different experiments; studying Qingjiang Rheumatic medicated wine's function of relieving pain by using the hot plate test and writhing test; observing its effects of anti-inflammation by experiment of testing a mouse's swollen auricle. Every experiment uses 40 mouse (half of them by gender), and divides them into 4groups at random as medicated wine in large dose group (7.8g/kg), in small dose group (3.9g/kg), Aspirin (0.2g/kg) and the Normal saline (20ml/kg).2. Clinical research: dividing 92 cases of such disease into two groups: one is Qingjiang Rheumatic medicated wine as treatment group, the other is Zheng Qing Feng Ning tablet as contrast group; adopting careful contrast of general therapeutic effects, main symptoms, sighs and experiment indexes to research this medicine's clinical effects and toxic reaction.Results: 1. Qingjiang Rheumatic medicated wine obviously increases the algesic threshold of the mouse by the hot plate test. Different doses of Qingjiang Rheumatic medicated wine and Aspirin can increase the algesic threshold of the mouse compared with the Normal saline group (p<0.01). While compared Qingjiang Rheumatic medicated wine with Aspirin, there exists little difference (p>0.05). Two groups in different dose have obvious effect of relieving pain within 30 minutes after using it, especially better for using large dose.2. Qingjiang Rheumatic medicated wine decreases the mouse's writhing times caused by injecting acetic acid into its abdominal cavity. Compared with the Normal saline group, two groups in different dose of Qingjiang Rheumatic medicated wine to decrease the mouse's writhing times have great difference (p<0.01), while compared with Aspirin, there has little difference (p>0.05), especially better for using large dose.3. The high and low dose of Qingjiang Rheumatic medicated wine can restrain from the mouse's swollen auricle caused by injecting dimethylbenzene. It restrains from the mouse's swollen auricle obviously by using a low dose of Qingjiang Rheumatic medicated wine and Aspirin. While compared with the Normal saline group (p<0.05), there has great difference. However, it is more obvious effect to use a low dose of Qingjiang Rheumatic medicated wine than Aspirin.4. Showed by clinical observation, the general effective proportion of such medicine curing rheumatoid arthritis accounts for 89.55%, higher than the contrast group (P<0.05). Its effect reducing arthralgia and bettering morning physical stiffness, the hands grip and joint swelling is superior to another group (P<0.05). However, two groups have no distinct difference in terms of 15 meters' walking speed. The symptoms and signs of two groups are superior to those before treatment (p<0.05). The three indexes of ESR, CRP, RF in cure group not only have marked distinction before and after treatment (p<0.05 or p<0.01), but also are distinct from another group (p<0.05). The IgG; IgA of cure group have differences before and after treatment (p<0.05). However,.another group has no clear distinction (p>0.05). In the course of treatment, there are neither clear side reactions, nor damage in hepatic function and renal function, except for few people feeling slight dizziness and stomach discomfort.Conclusion:The Qingjiang Rheumatic medicated wine cannot only relieve pain and resist inflammation well, but also has secure clinical effects on curing rheumatoid arthritis. At the same time, the medicated wine has little side reaction and it is safe and reliable in clinical application.
