BackgroundSkin and soft tissue absence is very commonly seen in hospital, such as decubital ulcer,abscess,infection of incisional wound,necrosis of flaps……The traditional saline-wet-to-dry dressings are used to treat the Skin and soft tissue absence wound to gain a proliferation of the granulation tissue for decades of years. Patients are suffering at the time of dressing change which may frequently performed. The leakage of fluid and wound odor also make the patient and his or her family very painful and inconvenient. With such a patient in charge, the doctor have to spend a lot of time in changing the dressings. What's more, the all manner of dressings and antibiotics used to control the infection are very expensive. An advanced therapy was needed very badly.The vacuum-assisted closure therapy was initiated by Doctor Fleischmann in 1992. It is an advanced drainage technique by placing an open-cell foam dressing into the wound cavity and applying a controlled subatmospheric pressure into the wound. It made an prominence contribution in granulation proliferation and had been used commonly in many departments like general surgery, plastic surgery, and department of orthopaedics ever since.We made a retrospective comparison of vacuum-assisted closure therapy and traditional saline-wet-to-dry dressings therapy in treating the Skin and soft tissue absent wound.MethodsFrom July 2007 to March 2008, a total of 37 patients with Skin and soft tissue absence wound was treated in our department. Eight of the thirty seven patients failed in traditional therapy and turned to vacuum-assisted closure therapy.20 patients underwent vacuum-assisted closure therapy and 25 patients were treated by traditional saline-wet-to-dry dressings. All patients were followed up for 1-8 months. We recorded and made a contradistinction of each patient's wound before and after the treatment, the time and money they spent, and their complain and satisfaction to the very treatment.Result1 eight of the thirty seven patients who failed in traditional therapy turned to vacuum-assisted closure therapy finally achieved healing.2 all the patients have not occurred complications.3 only one of the four diabetic feet recovered from vacuum-assisted closure therapy, others experienced amputations and transferred to another hospital.4 the healing time used in vacuum-assisted closure therapy is shorter than the traditional therapy.5 the proliferation speed of granulation tissue in vacuum-assisted closure therapy is more quickly.6 patients and their family members are more satisfied in vacuum-assisted closure therapy.7 the advanced therapy is no more expensive than the traditional therapy.8 compared with the traditional therapy the vacuum-assisted closure therapy spend less time.ConclusionsVacuum-assisted closure therapy is an convenient and effective treatment in treating the Skin and soft tissue absent wound. Compared to traditional saline-wet-to-dry dressings therapy, it can heal the wound more quickly, decrease the patient's pain, reduce the cost, and the most important is much more effective than it's comparer.
