By an analysis of prescriptions of traditional Chinese medicine written for ophthalmic in-patient in The Eye Hospital of China Academy of Chinese Medical Sciences in 2006, we made a research on rule and characteristic of therapeutic drugs in traditional Chinese ophthalmology (TCO) under the Guidance of Traditional Chinese Medicine Theory. We hope that this study would help to grasp the characteristic of drugs in ophthalmic treatment under Traditional Chinese Medicine Theory, and provide some assist and experience in clinical therapy.The study includes two parts. They are literature review and an analysis on rule of clinical drug using in TCO.Part 1 Literature ReviewThere are two parts in the literature review: one is a summarization of familiar diseases of TCO, another is a summarization of familiar Chinese traditional medicine and prescription of TCO. In the first part, we made a simple comment on clinical syndrome, cause of disease, pathogenesis, therapy and prescription of outer barrier (OB) such as Jiaochuang (trachoma), hordeolum, starred nebula (keratitis) etc., and inner barrier (IB) such as cataract, glaucoma and optic nerve atrophy etc. respectively. In the second part, we made an investigation and a review in the following three aspects: traditional Chinese medicine, prescription of traditional ophthalmology and experiential prescription of modern ophthalmology.Part 2 Analysis on rule of clinical drug using in Traditional Chinese OphthalmologyAfter a collection of 5,499 original prescriptions of in-patient in ophthalmology in The Eye Hospital of China Academy of Chinese Medical Sciences in 2006, we set up an Access data-base of drugs used in TCO by computer technology. Then analyze the rule of drug using in TCO under the guidance of traditional Chinese medicine Theory and Chinese traditional medicine Theory. In order to identify the types of diseases in in-patient, we separated the in-patient into two groups according to their diagnosis firstly, outer barrier (OB) and inner barrier (IB). The result showed: 235 patients belonged to OB, 995 patients belonged to IB, the total amount was 1,230, IB patients took up 80.89%. In the part of the analysis on rule of clinical drug using in TCO, we picked up the data from sorts of drug using, drug property, five flavors and channel tropism etc., and made a comparative study between the drugs used in OB and IB. The result showed: in term of drug property, more cold and cool drugs were used in the treatment of OB, it took up 53.16%, warm and hot drugs took up only 30.92%. The characteristic of drug using in IB especially for optic nerve atrophy was different from OB, cold and cool drugs took up 39.47%, warm and hot drugs took up 40.71%, this illuminated that the more drugs used in optic nerve atrophy were warm and hot. As for five flavors, bitter and sweet drugs were mostly used both in IB and OB. As for channel tropism, more ophthalmic drugs entered liver channel, but five zang and six fu could also be distributed at the same time. In terms of drug sorts, clearing heat and cooling blood drugs and breathing surface evil drugs were mostly used in the treatment of OB, and blood activating drugs and benefiting drugs were mostly used in the treatment of IB. As for familiar drugs, Radix Saposhinikoviae, Radix Rehmanniae Recens, Radix Paeoniae Alba, Rhizoma Atractylodis Macrocephalae, Radix Glycyrrhizae, Radix Angelicae Sinensis were commonly used in OB, and Radix Angelicae Sinensis, Radix Rehmanniae Recens, Radix Salviae Miltiorrhizae, Fructus Lycii were commonly used in IB. Radix Angelicae Sinensis, Radix Rehmanniae Recens, Fructus Lycii, Radix Salviae Miltiorrhizae were mostly used to benefiting and activating blood. The discovery of the rule of using benefiting and activating blood drugs in the treatment of IB, not only offering the good reference in ophthalmic drug, but also providing an evidence that liver is in charge of blood storage under traditional Chinese medicine, and eye can see by right of getting blood in .This study set up the Access data-base of drug in TCO for the first time, and made a discussion on the rules of drug using in TCO. Although this data-base needs to be optimized, and the data needs to be farther collected, to analyze the drugs using in TCO by the type of data-base should bestead to the development of Traditional Chinese Ophthalmology and clinical traditional Chinese medicine sciences.
