Professor Zhu Jiangui From The Spleen And Kidney On The Rule Of Geriatric Research

1.ObjectiveThe objective of this research is applying data excavating technology to reserch the diagnosing and differentiating discipline in the angle of spleen and kidney in gerontology from the experience of Doctor ZhuTinggui and to discover the geriatrician Zhujiangui's know-how of treating aged patients, and to learn the rules of Traditional-Chinese-Medicine(TCM) for geriatrics, from theory to clinical prescription.Hopefully,the results of expert experience can be repeated by others,hence they can improve the practice of TCM in geriatrics.2.MethodThis empirical research is based on the technique of data mining,and the method for clinical epidemics is the descriptive studies.The statistics focus on frequency analysis.To know the main types of TCM diagnosis for geriatrics,and to discover its clinical symptoms and its impact to TCM structure in prescriptions,this research uses the 8 elements of Zhengs, ie.clinical manifestation,to analysis the database of1296 cases of 600 patients aging above 55 years old,diagnosed and treated by Doctor ZhuTinggui. They are eight type syndromes of spleen-deficiency,Yang-deficiency of spleen, spleen-deficiency and phlegm-turbid stagnation,disharmonious of spleen-stomach,Yin-deficiency of kidney,Yang-deficiency of kidney, Qi-deficiency of kidney,kidney-deficiency and blood stagnation.3.ResultThe distribution of Zhengs:The truncated data set of aged 45 and over includes 1296 cases.The following is diagnosing ratio of syndromes:Most of all is Yin-deficiency of kidney 651 cases to the 50.2 percent;The second is(spleen-deficiency) phlegm-turbid stagnation 321 cases to the 24.8 percent; The third is(kidney-deficiency) blood stagnation 208 cases to the 16 percent; The forth is Yang-deficiency of kidney 163 cases to the 12.6 percent;Least of all is Yang deficiency of spleen 20 cases to the 1.5 percent.Among the total cases,the spleen-deficiency syndrome is 560 cases to the 43.2 percent while the kidney-deficiency syndrome is 1054 cases to the 81.3 percent.Also we develop the disease,formulae and desipline of using herbs relating to the eight syndromes of spleen-deficiency,Yang-deficiency of spleen,spleen-deficiency and phlegm-turbid stagnation,disharmonious of spleen-stomach,Yin-deficiency of kidney,Yang-deficiency of kidney, Qi-deficiency of kidney,kidney-deficiency and blood stagnation.The result is accord with the learning ideology and clinical experience of Doctor ZhuTinggui's therapy of gerontology in the angle of spleen and kiddney.Relations between Zbeng and symptom:The results of analysis of tonque and pulse for each Zheng are similar and not distinguishable,due to the decomposed data structure.Fortunately,other clinical symptoms make the 8 groups distinguishable from one another,hence those construct 8 series of symptoms for diagnosis.Relations between Zhengs and prescriptions:According to the frequency counted under Zhengs,the top 5 decoctions(and medicines)in Zhu's prescription for aged 45 and over are shown as following:①Yin-deficiency of kidney:(肾阴虚):知柏地黄汤zhibaidihuangwan,甘麦大枣汤ganmaidazaotang,杞菊地黄汤qi judihuangwan,酸枣仁汤suanzaorentang, duhuojishengtang.②Yang-deficiency of kidney:(肾阳虚):shenqiwan,wupiyin,ganmaidazaotang, jishengshenqiwan,duhuojishengtang.③Qi-deficiency of kidney:(肾气虚):shenqiwan,shenmaiyin,wupiyin, jishengshenqiwan,wulingsan,④spleen-deficiency:(脾气虚):buzongyiqitang,xiangshaliujunzitang, pingweisan,sinisan,sijunzitang,.⑤Yang-deficiency of spleen:(脾阳虚):liangfuwan,xiaojianzhongtang, lizhongwan,jinlingzisan,sijunzitang,.⑥kidney-deficiency and blood stagnation.(肾虚血瘀):huoluoxiaolingdan, duhuojishengtang,buyanghuanwutang,shenqiwan,shaoyaogancaotang.⑦spleen-deficiency and phlegm-turbid stagnation:(脾虚痰湿):pingweisan, wendantang,sanrentang,sinisan,simiaowan.⑧disharmonious of spleen-stomach:(脾胃不�'�):sinisan,banxiaxiexintang, liangfuwan,jinlingzisan,pingweisan.4.ConclusionApplying medical case database and data excavating technology in researching experts' typical cases are helpful to discover the internal rule so as to conclude the discipline of TCM experts' diagnosing and treating to some diseases.It is also a breaking-up to the TCM's traditional inheriting method and a new way and experiment of implementing the inheriting experts' learning ideologies and clinical experiences.
