OBJECTS This clinical observation included two groups:Ecthyma group and Sphacelus group. It has a total of 64 patients with chronic leg ulcers of 70 limbs. The same treatment plan for 2 weeks was put into practice on every patient which could be divided into two parts.One called basic treatment plan including invariable therapy for built-in diseases and vein drop therapy with Chinese medicine. The other called external therapy of changing dressings for ulcers with traditional ulcer Gauze which belong to Beijing University of Chinese Medicine Dongzhimen Hospital. Before and after the treatment we observed four indexes includes wound area, wound bacteriaes, wound granulation tissue and wound pain. Eventually we contrasted self-effects and effect power of interclass. To find out which group is more effective. And try to explain the possible mechanism of effection.METHODS By classification random sampling method we chosen 64 cases a total of 70 limbs from vascular branches of our hospital. One group of Ecthyma with 32 patients and the other group of Sphacelus with 32 patients too.Each group has 35 limbs. Before treatment the gerenral clinical data between the two groups has been well compared with(P>0.05). Except the same basic therapy, all wounds were changed with the Gauze dressing everyday for external local treatment for two weeks. We evaluated the wounds by scoring before and after the treatment. The data of two groups were analysis for self-control to clear whether the Gauze could improve wounds conditions significantly. And eventually we compared efficient between the two groups. Comparison of all calculated using the statistical software SPSS 13.0.RESULTSThe datas of two groups were significantly different before and after the treatment(P>0.05).The reduced area of the wound, wound bacteria inhibition, wound granulation promotion and wound pain relief are effective terms(P<0.05). Compared to the Sphacelus group, the Ecthyma group has a great range of wound area reduction and wound bacteria inhibition. After treatment the efficient of the two groups compared as significant difference(P<0.05). And the Ecthyma group is more effective (the Ecthyma 85.71%,the Sphacelus 48.57%).CONCLUTIONS The gauze can effectively reduce the area of wound; inhibit wound bacteria, promote wound granulation tissue and relief wound pain. For the purpose of this study, compared with the Sphacelus, the gauze is more suitable for the Ecthyma wounds. Some invalid wounds (Sphacelus 51.43%,Ecthyma 14.29%) initially determined to comply with negative wound. We consider if the treatment with oral medicine can reverse the predicament of these wounds to heal. Simple external agents can not solve the problem of all the wounds. We hope that people could give more attention to the oral medicine, and give necessary attention to the dialectical type wound.
