Rheumatoid Arthritis (RA) is one of the most common rheumatic diseases. The processes will drive the natural resolution of inflammation disrupted, and switch to a chronic persistent disease. Presentation with a new onset of inflammatory arthritis is remarkably usual. The damage to cartilage and bone occurs early and is due to synovial inflammation. And then some pathological changes such as the joint functional disorder, arthrocleisis, joint deformity and so on, can cause physical disability in the end. The main target of treatments is to alleviate clinical symptom, reduce the destruction of bone caused by the early synovial hyperplasia, and prevent the irreversible joint injury effectively. Therefore, it is very important that early diagnosis and treatment for the rheumatoid arthritis patients. But it is difficult to distinguish RA from other connective tissue diseases, which includes the systemic lupus erythematosus and primary sjogren syndrome at the same time.Rheumatoid factor (RF) is the earliest serological index for the diagnosis of rheumatoid arthritis, for its simple operation, high speed and sensitivity. But it has a low specificity. In recent years, it has been reported that the anti-cyclic citrullinated peptide (CCP) antibodies have higher specificity for the diagnosis of RA targets and course-related and patient both at home and abroad. The combination will greatly enhance the accurate diagnosis of RA.Objective:This study mainly analysis serological anti-CCP antibody and RF rheumatoid of arthritis patients and control groups, providing a accurate diagnosis and reasonable treatment for RA by seeking a reasonable diagnostic test methods and serological index progress of the Rheumatoid arthritisMethods:1. All Subjects were divided into case and control groups according to inclusion criteria. All patients were testing both for anti-CCP antibody and RF respectively, compared the differences detection rate between the patients with RA and the people without RA. Study the associativity by correlation analysis at the same time.2. The patients with RA were assessed on the scores by DAS28, the scores were compared with the concentration of anti-CCP antibodies, and then the associativity could be analyzedResults:1. The result ofχ2 test showedχ2=112.145, P<0.05, which has statistical significance. That means the positive relevance ratio of anti-CCP antibody is different in the patients and the control group. 2. The result ofχ2 test showedχ2=16.678, P<0.05, which has statistical significance. That means the positive relevance ratio of RF is different in the patients and the control group.3. With combined determination of RF and anti-CCP antibody:the sensitivity and specificity of the anti-CCP antibody is 70.25% and 91.5%; the sensitivity and specificity of RF is 76.86% and 47.06%; and the results in the series experiment and the parallel experiment are 61.16%,91.50% and 85.95%,47.06% respectively. There was no significant superiority in series experimental test than individual; the parallel assay could provide more diagnostic information than the single assay, contribute to an accurate diagnosis of RA.4. Anti-CCP antibody concentration and assessment of RA patients with DAS28 scores correlation R=0.907, they are highly correlated.Conclusion:There is significant deviation in the concentration of RF and anti-CCP antibodies between rheumatoid arthritis patients and non-patients. It is good for the disease testing and diagnosis. It can provide abundant clinical diagnostic information, to avoid the low specificity of rheumatoid factor, while improving the sensitivity of anti-CCP antibodies. These two kinds of methods are simple, fast, easy standardization, comparability of results, greatly improving on the accuracy of diagnosis of rheumatoid arthritis. Anti-CCP antibody concentration and assessment of patients with DAS28 scores has a good correlation, it can provide a reliable basis for clinical rational use of drugs. Combined Test of RF and Anti-CCP in Rheumatoid Arthritis give a satisfactory result.
