Background:The treatments to inferior turbinate hypertrophy with chronic rhinitis have medicines,operations,laser,microwave and radiofrequency ablation and radiofrequency ablation is a new technique in recent years. Radiofrequency ablation of inferior turbinate hypertrophy can make tissue of turbinate hypertrophy wrinkle by the plasma theory, characterized by the treatment without damaging the mucosal of inferior turbinate and superficial cilia and protect the mucosal physiological functions effectively. During the operation, we use superficial anesthesia on the mucosal, then inject the local anesthesics under the mucosal of inferior turbinate,this is the traditional local anesthesia. However, because of the specificity of the nasal anatomy, it commonly can not alleviate the pain completely during the operation only by local anesthesia. Furthermore, inject local anesthesics will change inferior turbinate's shape and lead to worse effect of RF ablation.It is a good method by acupuncture anesthesia during the operation, AA can be divided into acupuncture combined with GA,EA and local anesthesia, etc. AA has the characters of convenient economical,safe and no physiological disturbance., it can adjust physiological conditions, makes vital signs be stable and reduce the bleeding during the operation and has the rapid postoperation recovery. While AA is used in radiofrequency ablation of inferior turbinate hypertrophy, we can reduce or without local anesthesia, so that lighten the tissue edema, clean the operation area, decrease the complications and local anesthesics dangers, its analgesia effect can meet the needs of operations, improve the operation's quality and be suitable with the development trend of modern anesthesiology.Objective:AA can increased pain threshold and adjust the body's physiological changes and achieve analgesia purpose by stimulating some acupoints or body's specific sites. In this base, we can complete the certain operations or postoperation analgesia through supplemented by a few of drugs. In the area of pains treatments, AA could dredge meridians, regulate blood, balance yin and yang, relieve pain and other effects. But after a lot of clinic practices, it proved that single use Acupuncture could be completed by fewer cases of surgery, so that AA is only used in a few number of operations now. The purpose of my study is to find the feasibility of AA during inferior turbinate operation and has two points:a. achieve good anesthesia effectb. less affect vital signs and more patient satisfactionMethods:Cases:select 61 suitable elected operation patients were randomized into AA group and control group,31 in AA group and 30 in control group.Anesthesia:AA group:superficial anesthesia after 10 minutes electric acupuncture stimulating.Control group:common local anesthesia after superficial anesthesia.Operation:Two points RA to bilateral inferior turbinate by Celon Pro Breath Radio-frequency treatment instrument.Observation:Record drugs quantity, feelings of pain, SBP/DBP, HR in each record point; conditions of inferior turbinate in each record point before and post surgery; others:heart palpitations, chest tightness, nausea, comfort and an acceptable degree.Resulds:Results show in operation, pain level in AA group is lower than control group obviously and especially at left operation and R2; compared in each group shows that pain level is higher at left than right RF and L1,R1 have statistic significance(P<0.05), AA group's pain level is stable and left side in operation is lower than right.In circulation indications, BP and HR in AA group are lower than control group in operation, SBP at R2 and HR at L2 are different obviously (P<0.05) which are compared between group; compared in each group is indicated that circulation in AA group is stable, yet bilateral SBP,DBP in RF at R2 and L1,MBP in RF at R2 and L1 in operation are different obviously compared with circulation indications of 10 minutes after on bed.Indications of inferior turbinate has no statistic significance.No chest tightness, palpitations, short of breath in operation, patients are satisfied and acceptable.Conclusions:Acupuncture anesthesia for inferior turbinate hypertrophy with chronic rhinitis RA treatment has the characteristics of simple,economical,safe and no disturbance to physiological functions by drugs, can adjust physiological conditions and stable vital signs, no injection pains of local anesthesia,effect of analgesia can achieve surgery. Because of no local anesthesia, tissue edema is lightened, operation field is clear.Before this study, there is no report about AA used in inferior turbinate hypertrophy operations by searching relevant literatures. This study provides the scientific proofs of feasibility and rationality of AA used in suitable operations by clinic compared observations.
