| Objective: Based on statistics of patients diagnosed with allergic rhinitis(allergic rhinitis AR)in the Department of Otolaryngology,Dalian Central Hospital,according to whether they are accompanied by adenoid hypertrophy(AH),to determine the AH of AR patients The incidence and its relationship with age,and whether there is a certain relationship between the severity of AH and AR;to explore the improvement of allergic symptoms of AH with AR by anti-allergic drug treatment and surgical treatment,as AH and The diagnosis and treatment of AR provides statistical reference and basis.Methods: A total of 606 AR patients aged 3-12 years diagnosed in the Outpatient Department of Otolaryngology,Dalian Central Hospital from January 2020 to December 2020 were selected.The diagnosis of AR patients was based on the "Diagnosis and Treatment of Allergic Rhinitis in Children-Clinical Practice Guidelines2019" 》,using the German Allergy Screen system(Mediwiss,Germany)allergen detection system for specific allergen detection,using ELISA or enzyme-linked adsorption method to determine 29 major allergens,Ig E>0.35IU/ml is considered positive CBCT was used to conduct nasopharyngeal examination for AR patients,and AH patients with an A/N ratio greater than 0.71 were diagnosed.All patients were divided into two groups: patients with AR with AH(the first group)and patients with only AR(the second Group)to explore whether there is a connection between the severity of AR and AH.Grouped by age,respectively: 3-6 years old,7-9 years old,10-12 years old,by counting the number of AR patients suffering from AH in different age groups,comparing the difference in incidence rates between different age groups.The AR patients with AH who agreed to be treated in our department will be treated with anti-allergic drugs(Nesuna + Montelukast sodium chewable tablets)for 1 month,and the changes in the severity of AR and allergies in patients before and after drug treatment will be compared Relief of sexual symptoms.After drug treatment,a uniform endoscopic low-temperature plasma-assisted adenoidectomy was performed to compare the severity of AR and the relief of allergic symptoms with surgical treatment.Results: There were no significant differences in the four aspects of sex,age,age grouping,and whether there was a family history of AR in children with or without AR,and the difference was not statistically significant(p>0.05);from the aspect of symptom persistence From this point of view,AR patients with AH had a higher incidence of persistent rhinitis,and there was a statistical difference(p <0.05);among the 259 patients with AR with AH,197 patients with moderate to severe rhinitis(76.1%)),among the 347 patients with AR without AH,217 had moderate to severe rhinitis(62.5%).It can be concluded that the incidence of moderate to severe rhinitis in AR patients with AH is higher,and there are statistics The difference in physiology(p<0.05);for patients with AR and AH who were treated with anti-allergic drugs for one month,before and after the drug treatment,the allergic symptom score was lower than that before treatment,but there was no statistical difference(P>0.05).After surgical resection of adenoids,the scores of allergy symptoms were significantly reduced,with statistical differences(mild patients: P values were 0.001,0.010,0.001,0.005;moderate to severe patients: P values were all less than 0.001).And from the perspective of the severity of AR,drug treatment can improve the severity of AR(73.6before treatment and 71.7 after treatment),but the difference is not statistically significant.Plasma radiofrequency assisted adenoidectomy reduced the incidence of moderate to severe AR in patients(71.7% before treatment and 59.7% after treatment),and the difference was statistically significant(P<0.05).Conclusion: The presence of AH aggravated the severity of AR and increased the duration of allergic symptoms of AR.After removing the risk factor of AH(low-temperature plasma radiofrequency ablation(ELTPRA)under nasal endoscopy),moderate to severe AR occurred The rate is significantly lower than before treatment.Drug treatment can also reduce the incidence of moderate to severe AR,but the difference is not statistically significant compared with before the drug.Treatment of AR patients with AH: Compared with drug treatment,endoscopic low-temperature plasma radiofrequency ablation(ELTPRA)can effectively alleviate the patient’s allergic symptoms,reduce the patient’s allergic symptom score,reduce the severity of AR,and effectively improve The patient’s quality of life. |
