| BackgroudAt present, research has provided strong evidence supporting that inflammatory activation is an important pathway in the progression of heart failure. However the anti-inflammatory approaches so far are largely disappointing because of neutral findings or even a worsening heart failure. Therefore, the researchers are thinking that if there is a new anti-inflammatory target?In 2009, the study by T. Gombos for the first time showed direct multiple associations in heart failure patients between key vasoregulatory peptides and inflammatory molecules.the major novel fingding of this study is the presentation of strong correlation between ET-1 and ADM levels with inflammation in CHF.It may contribute to find new anti-inflammatory target.However, there are so many peptides in the Vasoactive Peptides family, and T. Gombos just select endothelin-1 and adrenomedullin in his research, which cannot indicate the popularity of the relationshio between Vasoactive Peptides and inflammation, so we need more further research on the other Vasoactive Peptides and inflammatory markers.Object:To study the change of the levels of adrenomedulin(ADM),BNP, hsCRP and ESR in the cohort of heart failure patients and to explore the relation between ADM,BNP and hsCRP,ESR。MethodFrom December 2008 to December 2009,60 heart failure patients were enrolled in the study who were admitted in the department of cardiology at the second xiangya hospital.According to the New York Heart Association functional class(NYHA),all the patients were departed into group 1 (NYHAâ… -â…¡) and group2(NYHAâ…¢-â…£).Asess the ESR and the plasma level of ADM,BNP and hsCRP.Result1. THE level of ADM,BNP and hsCRP between group 1 and 2 was significantly different. THE level of ADM(p=0.000),BNP(p=0.042), hsCRP(p=0.001) and ESR(p=0.000) in group 1 was higher than that of group 2 respectively.2. ADM was positively related with BNP,CRP and ESR respectively; and CRP had a positive relationship with ESR; NT-Pro-BNP was positively related with ESR and CRP respectively.3. The multiple linear regression for ADM and NT-Pro-BNP, after the adjust of confounding factors such as EF, shows that ADM and NT-Pro-BNP was positively related with ESR respectively.Conclusion1. inflammatory factors are activated in the heart failure patients. The worse the cardiac function, the higher the level of the ESR and CRP.2. vasoactive peptide was increased compensatoryly. The worse the cardiac function, the higher the level of NT-Pro-BNP and. ADM3. vasoactive peptide ADM and NT-Pro-BNP are possibly related with inflammation in the failure heart. |
