With the improving of people's living standard, people's life expectancy is increasing, and for most of the elderly people, especially the old old, will experience a long term chronic disease period, and thus physical disability happened, under this condition, care by other people in a long term need be provided for them, in western countries, it is called long term care (LTC). Most of western countries have experienced long term elderly welfare development, and comparatively well advanced social security system with three big support including pension, health care, long term care formed. In China, family has long been playing role providing long term care for the elderly people, however with the trend of ageing, family care function weakening, family care becomes much more limited, meanwhile, community and institution service for the disabled elderly people undeveloped, in the coming decades, the only child's parents will become elderly people, and there will have more long term care demand, and family, society, economy are facing more and more challenges of long term care definitely. So making analysis to the problems of the disabled elderly people and coping effectively will be critically important for the harmony society construction.With the background of ageing and welfare situation, this study focuses on disabled elderly people, through field study of family care and institution care, the author takes evaluation of elderly people's need, discusses dilemmas different parts facing, and give some advice on how to improve service.Study shows that disabled elderly people as the weak group, the main problem existed includes: short of family care resource, huge cost from medical and long term care leading to economy problem, mechanical barriers existed of institution running, short of community service for disable elderly people, so improve long term care service, including funding and monitoring need clearly role orientation for the government, community, family and individual or other parts in the future long term care system. The thesis has four parts in total: the first part includes the background, meaning, articles, concepts and methods. The 2nd and 3rd parts are material conclusion and problem summary from field study to family and institution care respectively, The 4th part includes summary to field study, the multi-role orientation in long term care welfare including government, community, family, individual, etc. and discussion on funding and monitoring of long term care service.
