Background:To assurance the effecacy of older living kidney transplantat ion,we must carefully sellect the older donors.In our country the living kidn ey transplantation start late,so we do not have enough experience.Obgective:Investigating the effecacy and safty of older living kidney transplantation.Methods:A retrospective analysis of 120 living kidney donors and recipients. Twelve of them were husbands and wifes,75 of them were parents and children and the other 33 were relatives. Fifty-two donors were over 55 years old. The living donors and recipients through the strict medical screening before operation, they all conform to the ABO blood group incompatible principles, both CDC and PRA were lower 0.1. And accord with ethics principle. All donors underwent open nephrectomy, the renals were placed in recipients iliac nest.Triple-combined immunosuppressive protocols consisted of calcineurin inhibitors (CNI, including CsA and FK506), MMF and steroid.The level of serum creatinine(SCr) before and after surgery and the out-break rate of complications were compared both donors and recipients.Results and Conclusion:Their kidney function recover well,The level of serum creatinine(SCr) after surgery had no significant difference between older and yonger donor groups; The SCr after nephrectomy was not different in recipients at one week,one month and one year. And the acute rejection (AR) had no significant difference between two groups too. Making a reasonable select standard is safe to older donor. It was also good for recipients, their kidney function recover well.
