The Clinical Research Of Decompensated Hepatitis B Cirrhosis Treated With Promoting Blood Circulation For Removing Blood Stasis, Detoxification And Inducing Diuresis

Objective:This study will investigate the clinical efficacy in the way of promoting blood circulation for removing blood stasis,detoxification and inducing diuresis,,by observing the change of the main clinical symptoms, positive signs and serum test, as well as discuss the optimal efficiency of combining Chinese with western medicine, to evaluate the effect in arresting the progress of decompensated hepatitis B cirrhosis,and observe the adverse effect toxicity and security.Method:30 posthepatitic decompensated hepatitis B cirrhosis patients will be involved from inpatient,who will be separated into the treatment group(n=15) and the control group(n=15) The latter take the normal and symptomatic treatment,and then the former adopt the method of promoting blood circulation for removing blood stasis,detoxification and inducing diuresis, with the latter's way. It will take one period of treatment (6 months) to observe the diversity of their the main clinical symptoms, positive signs and serum test.Result:Both groups can significantly improve the fatigue, anorexia, hypochondriac pain, abdominal distension, jaundice, bleeding, ascites symptoms (P<0.01), hepatosplenomegaly (P <0.05), slightly stronger role in the treatment group than the control group, and the control group, the treatment can significantly improve symptoms in patients with jaundice (P<0.01), fatigue, ascites symptoms (P<0.05); two TBil, ALT, AST, y-GT, AKP, CHE significantly after treatment improvement (P<0.01), compared with the control group, treatment group at 6 months significantly reduced AST (P<0.01) and TBil, y-GT (P<0.05); two albumin, prealbumin after treatment there are different degrees of improvement(P<0.01), compared with the control group, treatment group before the first 6 months were significantly higher albumin (P<0.01); Serum HA,Ⅲprocollagen and collagen type IV therapy significantly decreased after(P<0.01), compared with the control group, treatment group at 6 months of serum HA were significantly different (P 0.05); after treatment, PT and PTA have different degrees of improvement (P< 0.01), compared with the control group, the PTA group at 3 months (P<0.05) and 6 months (P<0.01) were significantly increased; two groups of HBV-DNA after treatment improved in varying degrees (P<0.01), compared with the control group, treatment group at 3 months,6 months, HBV-DNA levels decreased significantly statistically significant (P<0.05); the two groups after treatment in both AFP and CEA decreased, but the two sets of indicators to improve the tumor was no statistically significant (P>0.05); the thickness of the two groups after treatment, spleen, portal trunk diameter and V the depth of ascites were decreased compared with before treatment, compared with the control group, treatment group can significantly reduce the portal vein diameter of (P<0.01), significantly reduced ascites depth of (P<0.05). The thickness of the two groups to improve the spleen was no statistically significant (P>0.05).15 cases of clinical observation and treatment of hepatitis B patients with decompensated cirrhosis, the total effective rate was 86.7%, significantly better than the control group,15 cases of hepatitis B patients with decompensated cirrhosis 66.7%. In the clinical symptoms and signs, liver function, coagulation, hepatic-fibrosis three, HBV-DNA quantification, AFP, CEA and other serological tests, the treatment group was significantly better than the control group.Conclusion:By observing the change of the main clinical symptoms, positive signs and serum test, The treatment group is possessed of long-dated curative effect. Its effect in arresting the progress of decompensated hepatitis B cirrhosis is equivaqlent or better than western medicine.On the other way, the rate of the adverse effect is lower, and the security is higher.
