Purpose:To assess the ability of the DTT to show the fibers of the prostate in 3T magnetic resonance and determine the normative fractional anisotropy (FA) and apparent diffusion coefficient (ADC) values of healthy prostate.Materials and methods:Eleven volunteers with a mean age of 30 (25-44)years underwent 3T-MR imaging (MRI) and DTI using a 6-channel external phased-array coil,T2-weighted turbo spin-echo (TSE) axial images and DTI with single-shot echo planar imaging of the prostate were obtained. In Neuro 3D software, given the seed point of the interesting region and trace the prostate fibers, Regions of interest were defined in the peripheral zone (PZ) and central zone (CZ).ADC and FA values were determined,differences between parameters of the central and peripheral zones were assessed.Results:For all the patients the DTI sequence image and T2-weighted turbo spin-echo (TSE) axial images quality, the DTT clearly showing the prostate construction of the'"fibers".there was statistically significant difference in ADC and FA values between central and peripheral zones.The mean ADC value of the central zone (1.032±0.132x10-3mm2/s) was significantly lower than the mean ADC value of the peripheral gland (1.505±0.187x10-3mm2/s) (P<0.01).The mean FA value of the peripheral gland (0.1626±0.0227) was significantly lower than the mean FA value of the central zone (0.2385±0.0432) (P<0.01).Conclusions:DTI permitted good delineation of the prostate anatomy, correctly defining the capsule outline of the prostate. FA and ADC values can evaluating the diffusion of water molecules in prostate tissue. The quantity of FA and ADC values can reflect the prostate micro-structure organization difference. DTI in the evaluation of prostate pathologies have the potential for providing improved diagnostic information. Purpose:compare the diffusion tensor parameters of prostate cancer, negative normal regions,benign prostatic hyperplasia(BPH) and central zone and to incipiently determine the prostate diagnostic threshold of FA and ADC value;To assess the ability of the DTT to show the fibers of the prostate. Materials and methods:A total of 44 patients,include 16 prostate cancer patients with a mean age of 72 (57-84) years and 28 BPH patients with a mean age of 67.5 (47-84) years underwent 3T-MR imaging (MRI) and DTI using a 6-channel external phased-array coil,T2-weighted turbo spin-echo (TSE) axial images and DTI with single-shot echo planar imaging of the prostate were obtained. There are all histologically proved by TRUS biopsie or operation. In Neuro 3D software, given the seed point of the interesting region and trace the prostate fibers, Regions of interest were defined in the tumor regions, negative normal regions,BPH.ADC and FA values were determined,differences between parameters of the tumor regions,negative normal regions,benign prostatic hyperplasia were assessed.Results:there was statistically significant difference in ADC and FA values between tumor regions, negative normal regions,BPH.The mean FA value of the tumor regions (0.308±0.084) was significantly higher than the mean FA value of the negative normal regions (0.186±0.058) (P<0.05).The mean ADC value of the tumor regions (0.883±0.192×10-3mm/s) was significantly lower than the mean ADC value of the negative normal regions (1.727±0.320×10-3mm2/s) (P<0.05); The mean FA value of the tumor regions was significantly higher than the mean FA value of the BPH (0.203±0.029) (P<0.05).The mean ADC value of the tumor regions was significantly lower than the mean ADC value of the BPH (1.408+ 0.130×10-3mm2/s) (P<0.05); ROC cut off point of the ADC value was 0.725×10-3mm2/s, the diagnostic sensitivity was 100%, the specifieity Was 96%; ROC cut off point of the FA value was 0.311, the diagnostic sensitivity Was 100%, the specificity Was 68.7%;The DTT clearly showing the prostate construction of the"fibers".Conclusions:DTI and DTT in the evaluation of prostate pathologies have the potential for providing improved diagnostic information.
