Objectives: To investigate the relationship between TCM syndrome differentia- tions and the P15,ID4 gene promoter methylation status in the bone marrow cells of acute myeloid leukemia (AML) patients, and discuss the relationship between P15,ID4 gene methylation and the development of disease, to provide a basis for clinical diagnosis and syndrome differentiation of acute leukemia. Methods: Collected bone marrow from 35 cases diagnosed in the different subtypes with different stage of disease of AML patients,the patient according to the standard of the diagnosing types of TCM divided into:"deficiency of qi and yin"(16 cases),"Prevalence of toxic heat in the interior"(6 cases),"retention of phlegm- blood"(13 cases), another collection of 10 healthy bone marrow to do the normal control, extraction of bone marrow mononuclear cells. detection methylation status of P15 gene,ID4 gene promoter with Methylation specific polymerase chain reaction (MS-PCR) method , from the perspective of molecular biology to discuss the relationship between it and TCM Syndrome-type, and statistically analyzed the results. Results: (1) Positive rate of P15 and ID4 gene methy lation in"Prevalence of toxic heat in the interior","ret ention of phlegm- blood"and"deficiency of qi and yin"patients compared with the control group had significant difference(P<0.05), No significant pairwise differences (P>0.05),The positive rate of P15 gene methylation between the asthenia syndrome patients and the sthenia syndrome patients had significant difference(P<0.05). (2) M1, M2, M3, M4, M5 subtypes of AML patients appeared higher degree of methylation changes,The positive rate of methylation between each subtype had no significant difference (P>0.05).(3) The positive rate of P15 and ID4 methylation between the newly diagnosed patients and relapsed patients had no significant difference (P>0.05).(4) The positive expression P15 and ID4 gene methylation was associated with a higher white cell count(P<0.05),a higher percentage of bone marrow blast cells(P<0.05).(5) In 35 cases of AML patients, 25 patients had P15 gene methylation (positive rate 71.4%), 27 patients had ID4 gene methylation (positive rate 77.1%), compared with the control group there were significant differences(P<0.01).Conclusion:The positive rate of P15 gene methylation of the asthenia syndrome patients was higher than the sthenia syndrome patients, positive expression of gene methylation from molecular level showed the essence of TCM real memory;P15 and ID4 gene showed hypermethy lation of expression in leukemia cells with high white blood cell count, high proportion of bone marrow blast cells in the clinical characteristics, suggesting that P15 and the ID4 gene play a role in the pathogenesis of AML ,Contribute to the diagnosis and targeted therapy of AML.
