Endometrial polyps is a common and frequently-occurring diseases in women, it can cause menstruation estimates increase, abnormal uterine bleeding, infertility, and even a malignant transformation. But some patients have no obvious symptoms. There's a high incidence of endometrial polyps, especially since the hysteroscopy was applied widely in clinic. And a great many of endometrial polyps are improved, its incidence of EP seriously affect women's physical and mental health, especially for unexplained infertility patients, EP are probably the single pathogenic factors. So the research of pathogenesis and treatment of endometrial polyps is important to solve the problem. It will benefit the growth period and menopausal women. At present, there are many scholars have over-indulgence its pathogenesis, and puts forward a lot of relevant theories, and expect a better clinical treatment effect. Most scholars believe that:endometrial polyps are estrogen dependence disease, and is closely related to related estrogen metabolic abnormalities, long-term single estrogen stimulation causes endometrial reactive hyperplasia. And scholars think endometrial polyps are occurred by abnormal endometrial apoptosis; the traditional ideas are that long-term chronic inflammatory stimulation causes inflammation stimulating factor expression, and lead to increased endometrial chronic inflammation reactive hyperplasia. In addition, some scholars advocate the theories of endometrial polyp immune abnormalities and the theory of genetic predisposition. In the actual clinical work, patients suffering from estrogen dependence disease whose serum hormone levels are in the normal range, thus we speculate that the happening of the estrogen dependence disease is not merely connected with the hormone levels in blood circulation, and the more possible factors related with local estrogen level of small environment concerned. Some scholars research also support the idea that the estrogen in blood circulation is high, but local target cells play function. At present, each versions of endometrial polyps the pathogenesis is still no unity of understanding.ObjectiveBecause the experiment technology of direct determination of estrogen concentrations in local target cells is difficult to implement, so we study the two other factors of influencing local estrogen metabolism:17beta hydroxyl steroid dehydrogenase 2 and progesterone receptor in normal endometrium and endometrial polyps tissue, and analyzes its pathogenesis in endometrial polyps, indirectly analysis the significance of the estrogen metabolismlocal in endometrial polyps. Hope that we can find the related factors of endometrial polyps possible outbreak from the endometrial estrogen local metabolism, excuse a new ideas for clinical treatment, or provide theoretical support to the research and development of drugs.MethodsThis study collected in first affiliated hospital of Zhengzhou University reproductive medicine center hysteroscopy check infertile women of reproductive age of endometrial tissue, divided into research group (endometrial polyps group) and the control group (normal endometrium tubal obstruction or by groups:the male factors of help pregnant patients), infertility treatment per patient collect menstrual 2 to 4 days, according to the pathological serum installment will study group is divided into proliferation period and combination, namely secretory respectively the normal endometrial proliferation period is organization and secretory endometrium, value- added period of normal tissue and polyp, using the electrochemical luminescence polyp organization in the serum concentration determination of estrogen, immunohistochemical determination method of different time respectively within the organization and beta hydroxyl steroid dehydrogenase 2 and progesterone receptor extremity, the statistical methods of expression in time don't analysis of endometrial and endometrial polyps tissue of expression differences.Result(1)The general condition of the patients age, body mass index, infertility fixed number of year, menstrual cycles, is no difference (P>0.05). (2) The six hormone levels of patients in 2-4 day's menstruation no significant difference (P>0.05). (3) HSD2 17 beta is a small film in proliferation period, and the expression in polyps group in the proliferation period is markedly reduced (P<0.05), and the proliferation of 17βHSD2 in endometrial glands epithelial cells are relatively obvious, the stromal cells rarely express; There are significant 17 beta HSD2 expression in the glands in secretes period and stromal cells,17 betaHSD2's expression in secretory tissue polyps are is abated, the expression in terstitialabate obviously (P<0.05). (4) The expression of the progesterone receptor B in the glandular epithelium and stromal cells of polyps are decline than normal endometrium, and in the stromal cells the express decline more apparently. (P<0.05).ConclusionsIn this study, endometrial polyp's patients and endometrial normal's age, body mass index, infertility fixed number of year, the menstrual cycle is no difference, and the patient's serum basic hormone level is no difference. Patients have no obvious disorder of hormone. Moreover, endocrine metabolic disorder can not explain the endometrial polyps happen separately.17 beta hydroxyl steroid dehydrogenase 2 and progesterone receptors in the gland epithelial and stromal cells of polyps are relatively less, the expression leads to high active estradiol metabolic rate of decline for female ketone, lining local estrogen relative overmuch, and progesterone receptor extremity content drop in progestational hormone cannot effectively play antagonists endometrial proliferation estrogen stimulation, the two factors the role of the role of strengthening each other ultimately led to the occurrence of endometrial polyps, therefore, this research think:endometrial local 17 beta HSD2 and express reduce extremity is endometrial polyps possible factors.
