Objective:Through observing the effect of Hyaluronic Acid Nuxvomica Total Alkaloids Gel Solution on articular cartilage's pathological morphology and matrix metalloproteinasel(MM P-1),interleukin6(IL-6), tumor necrosis factor-a(TNF-a) contents in Knee synovial fluid and the expression of matrix metalloproteinasel3(MMP-13) in cartilage, we initially studied the curative effects and mechanism of Hyaluronic Acid Nuxvomica Total Alkaloids Gel Solution on prevention of KOA.Methods:(1)We established the rat KOA model through injecting 4% papain solution into joint cavity, the normal group was not modeled, the model rats were divided into six groups, which wasnormal sodium group(NS),menstruum group,sodium hyaluronate group(SH), high dose of Hyaluronic Acid Nuxvomica Total Alkaloids Gel Solution group and commonly used dose of Hyaluronic Acid Nuxvomica Total Alkaloids Gel Solution group(Hereinafter referred to as high dose of Strychnos alkaloids group, commonly used dose of Strychnos alkaloids group.). In addition to the normal group, we respectively injected normal sodium injection, menstruum injection, sodium hyaluronate injection, high doses of Strychnos alkaloids injection and commonly used dose of Strychnos alkaloids injection into the model rats'joint cavity, intervented once a week, after five weeks, got rats'bilateral knees to observe and detect;(2)We used naked eyes to observe the change of articular cartilage's general morphological changes and utilized light microscope to observe the change of articular cartilage's Pathological changes after HE and AB-PAS staining;(3)Respectively, Elisa and radioimmunoassay methods were used to detect IL-6, MMP-1, TNF-a levels of the knee joint fluid;(4)RT-PCR was performed to observe the effect of Hyaluronic Acid Nuxvomica Total of Hyaluronic Acid Nuxvomica Total Alkaloids Gel Solution on the expression of MMP-13 mRNA in articular cartilage of the rats with KOA.Results: (1)Observation of general morphology of the knee:The rats in normal group without degenerative changes in articular cartilage and synovial membrane without inflammatory changes, too; The rats in normal sodium and menstruum group both appeared that the synovial membrane was hyperplasia severely, the hydrops of intra-articular was obvious, articular cartilage was gray dull color, they appeared that the color was gloomy, cartilage surface cracked, defected, severed deep bone, some edge of cartilage formed visible osteophytes; SH group and high dose of Strychnos alkaloids of rats with moderate hyperplasia, part of the rats' articular cavity had sticky hydrops, articular cartilage thinning, slightly rough, the surface brightness was not obvious, we could see a little point-like depression; commonly used dose of Strychnos alkaloids group showed that synovial membrane appeared mild hyperplasia, little hydrops, cartilage was slightly smooth, with scattered point-like depression.(2)Articular cartilage's pathology morphous aspect:Analyzed the score points of HHGS of each group, we could see that after two weeks of modeling, the model group compared with the normal group was stastically significant(p<0.05), the model group was higher than the normal group. After drug intervention period, high dose of Strychnos alkaloids group, commonly used dose of Strychnos alkaloids group, SH group compared with NS group and menstruum group had significant differences(p<0.05), the former three was significantly lower than the latter two; the commonly used dose of Strychnos alkaloids group compared with high dose of Strychnos alkaloids group were also significant differences in contrast (p<0.05), the former was significantly lower than the latter two; high dose of Strychnos alkaloids group and SH group were no significant difference(p>0.05).(3)Knee synovial fluid MMP-1, IL-6, TNF-αcontents detection:High dose of Strychnos alkaloids group, commonly used dose of Strychnos alkaloids group, SH group compared with NS group and menstruum group had significant differences(p<0.05), the former three was significantly lower than the latter two; the commonly used dose of Strychnosalkaloids group compared with high dose of Strychnos alkaloids group were also significant differences in contrast (p<0.05), the former was significantly lower than the latter two; high dose of Strychnos alkaloids group and SH group were no significant difference(p>0.05).(4)Articular cartilage MMP-13 mRNA expression:High dose of Strychnos alkaloids group, commonly used dose of Strychnos alkaloids group, SH group compared with NS group and menstruum group had significant differences(p<0.05), the former three was significantly lower than the latter two; the commonly used dose of Strychnos alkaloids group compared with high dose of Strychnos alkaloids group were also significant differences in contrast (p<0.05), the former was significantly lower than the latter two; high dose of Strychnos alkaloids group and SH group were no significant difference(p>0.05).Conclusion: (1)The high dose of Strychnos alkaloids group, commonly used dose of Strychnos alkaloids group and SH group could improve the KOA papain induced pathological articular cartilage and reduce the articular cartilage degeneration, damage and synovial inflamemation, which commonly used dose of Strychnos alkaloids of intervention is most significant.(2)The possible mechanism of injecting Hyaluronic Acid Nuxvomica Total Alkaloids Ge 1 Solution to prevent and treat rat knee osteoarthritis model which is induced by papain:①Reduce the knee joint synovial fluid IL-6,TNF-αlevels, thereby reducing its mediated arthritis and injury;②Reduce the knee joint synovial fluid MMP-1 level and MMP-13 mRNA expression, reduce the degradation and damage of articular cartilage, thus maintaining the stability of articular cartilage matrix.
