Research On Normalization The Type Of Kidney Deficiency And Blood Stagnation Of Primary Nephrotic Syndrome In Children

Objective:Pass the method of "Dialectical system of type's elements "and KY3-H Dialectical system of type's elements, analysis the data means of child primary nephrotic syndrome by four methods of examination. To approach the distribution of the pathological part and patho-character,the principal syndrome-types in TCM. Explore the main diagnostic points in type of Kidney Deficiency and Blood Stagnation initially, the relation between type of Kidney Deficiency and Blood Stagnation and clinical correlated test. To establish the bases on norm the symptom diagnosis standard in type of Kidney Deficiency and Blood Stagnation.Methods:190 children patients with PNS from The people's hospital affiliated to Fujian TCM university and Fuzhou General Hospital of Nanjing Military Region from December,2009 to February,2011. Collecting the information of children patient by four methods of examination, Examineing TXB2,6-keto-PGF1αand platelets,complement C3,complement C4,IgA,IgG,IgM,bio-chemical(Renel function,Proteinum and Lipide),ESR,HCY,urineβ2-MG,24 hurine protein quantity,D2-dinner,FIB etc.The four methods of examination is analysised by the system of KY3-H differentiation of symptoms and signs, then abstraction and test index of pattern of syndrome was performed by SPSS 15.0system software.Results:1. The part of pathological changes with PNS involve in 11 items, such as kidney (88.4%),spleen (74.21%),lung(44.21%),muscle and skin (20%),surface(14.7%),stomach (9.47%), ect. The character of pathological changes with PNS involve in 21 items, such as Water accumulating in the interior (97.9%),Qi deficiency(78.4%),Heat (52.6%),Wetness (44.2%),Yang deficiency(33.68%),Blood stasis (30%),Qi stagnate(12.11%),exogenous wind-evil (11.6%),Yin deficiency (9.47%), ect.2. There are hundreds of syndrome-types in TCM with PNS on the clinical, the top five are type of Deficiency of kidney and Water accumulating in the interior (92.6%),Deficiency of spleen and Water accumulating in the interior (90.5%),Wetness-muddy Distressing spleen (43.2%),Wetness-heat accumulating in the spleen (33.2%),Kidney Deficiency and Blood Stagnation (28.9%). There are three grades in type of Kidney Deficiency and Blood Stagnation, the second grade accounts 30.91%, the grade between the second and the third accounts 50.91%, the third grade accounts 18.18%. 3. Succeed to to set up the diagnosis model on type of Kidney Deficiency and Blood Stagnation by logistic stepwise regression, the prognostic accurate percentage achieves 93.7%.4. The ratio of TXB2/6-keto-PGF1a disequilibrated on children with PNS. There are significant deviations on the consequence of TXB2,6-keto-PGF1α,IgG,TP,ALB,TC,UA between type of Kidney Deficiency and Blood Stagnation and other types of syndrome (P <0.01andP<0.05).Conclusion:1. Kidney,Spleen and Lung are the chief parts of pathological changes on children with PNS, Water accumulating in the interior,deficiency(Qi deficiency and Yang deficiency),Wetness-heat and Blood stasis are the are the chief characters of pathological changes on children with PNS.2. The Symptom of "Kidney Deficiency and Blood Stasis" in TCM is actually existence in clinical on children with PNS, the type of Kidney Deficiency and Blood Stagnation is one of the hackneyed types.3. Pain-immobilization,abdomen bulge,ascites,faces and eyelids edema,edema limitations,paining refusing pressed or pain much more when pressed,easily to catch a cold,silkiness red spots etc are the main symptoms of the type of Kidney Deficiency and Blood Stagnation.4. Pain-immobilization, refusing pressed or pain much more when pressed; ascites or abdomen bulge or edema or face and eyelids edema or local edema; Shen wearied or easily to catch a cold; skin pigmentation or silkiness red spots; nodus or lumps which are hard and not flat; weak sudor or easy to perspire; dark tongue or moulage on the tongue or coated tongue defect/few/none or collaterals varicose under the tongue or speckle on the tongue, these are the main symptoms of the type of Kidney Deficiency and Blood Stagnation, it could be primary diagnosis the type of Kidney Deficiency and Blood Stagnation.5. Examineing TXB2,6-keto-PGF1α,IgG,TP,ALB,TC,UA is valuable to distinguish the type of Kidney Deficiency and Blood Stagnation.
