As directly related to the therapeutic efficacy and the security of life, the safety of surgery has been a common concern of both doctors and patients. It is one of the duties for both medical institutions and personnel to prevent the occurrence of surgical safety issues. World Health Organization, governments and hospitals at all levels have taken many measures to solve surgical safety issues, but the situation remains not optimistic.Hazard analysis and critical control point (HACCP) is the most authoritative food safety and quality assurance system which has been strongly recommended by Food and Agriculture Organization of the United Nations (UNFAO) and WHO since the late 1980s. HACCP has been widely used in the safety control of food, aquatic products processing and other industries and achieved significant effect. Rather than the traditional end-product testing system, HACCP is a preventive system that can make hazard analysis of the process, control the key factors and then provide security for the result. HACCP system has the following advantages:emphasizing the identification and identification, integrity scientific evidences, traceable records, making the reasonable potential hazards to be identified, keeping coordination with the quality management system.PurposeThis study is based on the catholicity and the severity of the safety of surgery, which is a both international and internal problem, and the characteristics of HACCP. It uses questionnaire to survey the staff in operating room and analyze their cognition of the safety problems of surgery and their existing problems. HACCP system is introduced to the safety management of the surgical patients. The aim is to establish a firenew security system for the surgical patients.Materials and MethodsIn this study, a questionnaire was designed chiefly, the content of which included the general state of the respondents, cognition of the importance of the safety of surgery, measures taken by the current hospital to ensure the safety of surgery for patients, and the possible security risks of surgery. The questionnaire was mailed to the staff in operating room of eight third-grade hospitals in certain province, and the finished data was coded and entered into the computer to create an excel database. Software SPSS 11.0 was used for statistical analysis.This study established HACCP system in the safety management for patients in operating room in certain third-grade class-A general hospital in Shandong Province and organized HACCP team. The team members and the staff in operating room received certain training on HACCP associated knowledge. We drew up the flow charts, analyzed the hazard factors, determined the critical control points, formulated the HACCP schedule and checklist, set up monitoring program for the critical control points, and formulated corrective actions.Results and Analysis98.66% of respondents believed that the safety management of surgical patient is very important. Many respondents said the sick had wristlet identification (77.85%), surgical patients safety checklist " (97.32%), surgical site identification systems (77.85%), emergency response plans (97.99%) and other safety measures in their hospital. Some respondents believed that security risks presented in the transferring of patients undergoing surgery(80.54%), pathological specimens management40.94%), body position(87.25%) and other aspects.This study set up a HACCP working group consisting of 6 person in operating room in certain third-grade class-A general hospital. The HACCP working group formulated team responsibilities, leader responsibilities and members responsibilities; made HACCP training and examination, drew flow charts about the delivery, surgery and safety check of the patients, operative instrument inventory, sending pathological specimens, blood transfusion, body position and infection control; analyzed the hazard factors; determined the CCP of the safety of surgical patients:the function of vehicle for transferring, the process of transferring patients, checking the patients, item inventory, checking blood transfusion, sending specimen, making body position, and infection control; formulated the HACCP schedule and checklist.RecommendationsTo establish HACCP system in the safety management for patients in operating room must be ratified and supported by the management and need a lot of staff and additional time, resources and capital investments. Implementation of the HACCP system should base on its own reality, roundly measure the management, workflow, environment, professional features and other aspects, make suitable reconstruction measures to achieve the purpose of hazard control. Note that HACCP system only to aim directly at the significant hazards, rather than trying to control all hazards. Good work system of the hospital, reasonable personnel structure and the professional quality are the basis for the safety of patients and successful implementation of HACCP. Only combined with the medical rules and regulations organically, the HACCP system may become a complete system for the safety of patients. After the establishment of HACCP system, it must be effectively implemented. Hazard analysis, identification of critical control point, determination of critical control limes are parts of the research phase of HACCP. Subsequently, the monitoring systems for each critical control point, verification procedures, establishment and application of records and document retention systems are parts of the implementation phase HACCP. The HACCP systems can either not replace the current medical rules and regulations.
