Objective:To investigate the changes of C—reactive protein (CRP),white blood cell(WBC) counter and neutrophil granulocyte ratio(N%) in acute exacerbations of chronic obstructive pulmonary disease(COPD); at the same time, CRP was measured when the patients after the treatment with effective antibiotics and compared its change. And to evaluate the clinical implications of C—reactive protein (CRP)in acute exacerbations of COPD.Methods:Subjects were divided into two observed groups:AECOPD group included 62 patients with acute exacerbations of chronic obstructive pulmonary disease, The serum CRP levels, WBC counter, N% before the treatment and CRP levels affter the treatment were detected of the same 62 inpatients with acute exacerbations of COPD, and WBC counter and N% were detected from 36 cases of normal volunteers (control group).Results:(1)The levels of CRP for patients of AECOPD group is (55.18±60.60)mg/ L,the normal level is 0-10mg/L, AECOPD group were higher than control group's highest level 10mg/L (P<0.05). The WBC counter and N% for patients of AECOPD group were significantly higher than control group ((9.71±5.51)×109/L versus (6.63±1.56)×109/L, P<0.05; 74.37%±12.12% versus 55.35%±8.80%, P<0.05; respectively).(2)The sensitivities of CRP, WBC counter and N% were 80.65%,30.64%, 62.90%, respectively. The sensitivities of CRP is higher than the sensitivities of WBC counter and N%(P<O.05). And the sensitivities of N% is little higher than WBC counter (P<0.05).(3) The levels of CRP for patients of afert the treatment were (24.53±24.01)mg/L,which were lower than before the treatment (P<0.05). So the level of CRP is a sensitive parameter to detect bacterial infection for AECOPD patients, provides early markers discover the acute exacerbation of COPD, and the CRP level is a good guide of the use of antibiotics.Conclusion:1. CRP,WBC counter and N% measurements in patients of chronic obstructive pulmonary disease play a role in the diagnosis of acute exacerbation of chronic obstructive pulmonary disease.2. CRP is better than WBC counter and N% in the diagnosis of acute exacerba-tion of chronic obstructive pulmonary disease. And N% meybe better than WBC counter.3. CRP is involved in acute inflammation process in acute exacerbation of COPD, so the level of CRP is a sensitive parameter to detect bacterial infection for AECOPD patients, provides early markers discover the acute exacerbation of COPD, and the CRP level is a good guide of the use of antibiotics.
