Objective:To research the role of cytokines sVAP-1 and sICAM-1 in diabetes mellitus patients with lower limb angiopathy.Methods:①Study subjects were consecutively collected from in-patients of endocrine secretion and departments in hospital from Jan 2010 to Nov 2011, and healthy individuals were selected from health examination center. Subjects with diabetes with acute complication, serious cardiac, liver, kidney and cerebrovascular diseases, acute inflammation, autoimmune disorder, resistant hypertension, type 1 diabetes, and cancer were excluded from this study.②A case-control study was conducted in total 84 subjects. Patients were divided into three groups with matching age and gender, healthy controls (20 subjects), excluding vasculopathy all of the body by Doppler ultrasound scanning; Type 2 diabetes individuals (64 subjects) with anti-diabetes medication in hospital, type 2 diabetes was defined by using the criteria of international diabetes federation. Subjects with ketosis, peripheral vascular disease and embolism and using anti-inflamation medication were excluded in this group. According to whether complicating lower limb angiopathy, B group was dvided into two subgroups(B1, without lower limb angiopathy and B2, with lower limb angiopathy), B1 group,30 subjects excluding lower limb angiopathy by Doppler ultrasound scanning; B2 group,34 subjects with endomembrane thickening, angio-plaque or thrombogenesis.③linical examination: Data regarding medical, social, and course of disease were obtained during an interview. Height, weight, blood pressure, BMI, plasma glucose TG, TC, HDL-C, LDL-C, HbA1c, plasma insulin and C- peptide were measured;④The serum sVAP-1 and sICAM-1 were performed by immunoenzymatic ELISA kit. All the results were analyzed with SPSS 13.0 for windows. Results:(1) The serum sVAP-1 and sICAM-1 and CRP level in B2 group was significantly higher than that of control (p<0.05). The serum sVAP-1 and sICAM-1 and CRP level in B2 group was higher than B1 group (p<0.05). The serum sVAP-1 and sICAM-1 and CRP level in Bl group was higher than that of control (p<0.05).(2) There was a linear correlation between the serum sVAP-1 and sICAM-1 and CRP level.(3) Multiple logistic regression analyses showed sVAP-1, CRP, sICAM-1, HbA1c were the risk factors of incidence diabetes survey lower limb angiopathy.Conclusions:(1) The high expression of sVAP-1 and sICAM-1 have intimate relation with the development of diabetes of lower limb angiopathy.(2) sVAP-1 and CRP maybe used as index of early diagnosis of lower limb angiopathy disease in type 2 diabetes mellitus.
