Object: To investigate the dynamic changes and significance of cortisol(Cor),adrenocorticotrophic hormone(ACTH),malondialdehyde(MDA),superoxide dismutase(SOD) in serum;glutathione(GSH) in whole blood and glucocorticoid receotor(GR) of lung tiddues after firearm wound in livers of rabbits in high altitude.Aimed at enhancing the integrated treatment of liver injury in high altitude,and reducing the mortality rate of liver injury,and also providing a theoretical argument for the prevention and treatment of multiple organ dysfunction after firearm wound in high altitude.Methods: One hundred and forty-four new zealand healthy and adult rabbits were divided randomly into: normal environment group(groupA),newly entering high altitude group(groupB),adaptation to high altitude group(groupC),and each group were subdivided into control group and 0.5,2,6,12,24,48,72h after firearm wound,n=6.After successfully eatablished firearm wound model of livers in different attitudes,blood samples and lower left lung tiddues were collected respectively at diffierent time points,Cor and ACTH in serum were detected by ELISA;MDA content in serum were detected by thibabituric acid method;SOD activity in serum were detected by xanthine oxidase method; and GSH content in the whole blood were detected by colorimetric quantitative method;lung GR expressions were measured with immunohistochemical staining and image analysis.Results:①Compared with control group,after firearm wound in the liver,Cor,ACTH content in rabbit serum of each group rose gradually,and reached the peak at 6h(P<0.01),12h was still at a high level,and then declined progressively;MDA content rose gradually after injury,and reached the peak at 12h(P<0.01),and declined gradually,72h was still higher than normal,And SOD activity and GSH contect slightly rose early after injury,and then gradually declined until reached the minimun,then progressively rose;GR expressions began to decline after injury,the lowest point was at 6h and then progressively rose,72h was still lower than normal level.②Compared with normal environment group,Cor,ACTH contect of newly entering high altitude normal group was higher than plain normal group (P<0.01),the extent of rise and maximum level was also more salient than normal environment group after injury;And the extent of MDA content reached the peak and SOD activity,GSH contect rose early and then reached the low point was also more salient;And the extent of decline and minimum level of GR expression was also more salient(P<0.01).③Compared with normal environment group,adaptation to high altitude group was more salient about the extent of rise and maximum level of Cor,ACTH contect,and the rise extent of MDA content and the decline extent of SOD activity,GSH contect (P<0.05),and the extent of decline and maximum level of GR expression,but their extent was lower than the newly entering high altitude normal group.Conclusions:①After firearm wound in livers in high altitude,under the double stress of trauma and altitude hypoxia,the body produced a significant stress response,and the extent of stress was stronger than plain traumatic stress.②Firearm wound break the balance between the oxidative and antioxidant,altitude hypoxia enable the body to produce too many free radicals and decrease the ability of the body to eliminate free radicals,oxygen radical may be involved in the body pathophysiology process after firearm wound.③GR downregulation might cause to inhibit their function, particularly reduced the anti-inflammatory effects,resulting in systemic inflammatory response syndrome,and might be the important reason of multiple organ failure.
