Objective To explore the correlation between seizure and metabolic dysfunction of neuron and glia cell in the hippocampus by MRS in children and adolescents with temporal lobe epilepsy (TLE) ,and to improve our understanding of progressive changes and the underlying mechanisms in the focal temporal lobe epilepsy. And a preliminary study of secondary epilepsy with hippocampal sclerosis may exist between the relationships.Methods and Materials 38 teenagers and children patients were diagnosed as TLE by comprehensive use of clinical history and neurological examination, scalp EEG with sphenoidal electrodes, conventional magnetic resonance imaging (cMRI); There were 23 men and 15 women with the age of 7 to 22 years (mean age, 12.7 years). 10 healthy volunteers with known history of no neurological disease were recruited as control. Choose independent sample t-test method, we think P≤0.05 significant statistical significance.Furthermore,analysis was performed in the correlation of seizure and duration with the ratio of NAA/ (Cho+Cr) in the hippocampus ipsilateral to epileptogenesis foci and contra lateral to epileptogensis foci; Another 10 cases of secondary with epilepsy, to analyze the metabolites between secondary bilateral hippocampal between with normal controls have obvious difference.Results The ratio of NAA/ (Cho+Cr) was significantly decreased in the head, body and tail of the hippocampus in the TLE compared to the control group. But in the TLE patients, no significant correlation was found between the seizure and duration with the ratio of NAA/ (Cho+Cr) in the hippocampus ipsilateral to epileptogensis foci, and neither was in the hippocampus contra lateral to epileptogensis foci; Secondary epilepsy with bilateral hippocampal NAA / (Cho + Cr) values was decreased compared with normal control group, and the metabolite ratio decreased more obvious lesion side and location within the brain parenchyma was no significant correlation. The ratio of NAA/ (Cho+Cr) tends to increase along both hippocampi in the poster anterior direction in patients with TLE and in the control.Conclusions Whether temporal lobe epilepsy or secondary epilepsy, our findings suggested that neuronal cell loss or dysfunction may occur in the hippocampus ipsilateral to the epileptogensis foci, but the worsening of epilepsy disease was not in agree with the seizure frequency and the duration of epilepsy history. Above suggest temporal lobe epilepsy of children and young people early on in the hippocampus of epileptic formed pathological damage, the experiment failed to prove that such damage has a cumulative effect, and this damage is progressive, or whether the process of development by leaps and bounds, still need to discuss further integration of adult epilepsy. .
