ObjectiveThe purpose of this study is to explore the relationship and explicit clinical significance between the levels of Oxidized low density lipoprotein(OX-LDL) and soluble p-selectin (SP-S), and Carotid atherosclerosis (CA) in Metabolic syndrome (MS) patients. The goal is to provide clinical basis for the prevention and treatment of Carotid atherosclerosis with MS patients.MethodsThe datas were selected from the outpatients and inpatients who have perfect clinical informations in cardiovascular sections of affiliated hospital of Hebei United University from February 2010 to June 2010, and totoly 86 eligible MS patients was selected. MS were divided into A group,which have three components of MS diagnostic criteria , and B group,which have four or more components of MS diagnostic criteria. The contral group'data were selected from the checkup sections of affiliated hospital of Hebei United University,and 39 cases healthy subjects who have eligible datas were selected. The blood samples of subjects were obtained from the antecubital vain after an overnight fasting period more than 8th for measurement of serum Total cholesterol (TC),Triglycerides (TG),Low-density lipoprotein cholesterol (LDL-C),High-density lipoprotein cholesterol(HDL-C),Fasting blood glueose (FBG),Creatinine (CR),Urine acid (UA),Blood urinary nitrogen (BUN)in the morning. Serum OX-LDL and SP-S was detected by enzyme-linked immunosorbent assay (ELISA). The measurement of carotid artery Intima-media thickness (IMT) and carotid artery plaque (CAP)of the subjects were performed used the ACUSON Sequoia 512 ultrasound Doppler with 10.0-14.0 MHz mainframes transduced by the same physician.Datas were put into the computer with the software of Excel 2000 and analyzed with the SPSS 13.0 software. The count datas were expressed as a percentage and compared byχ2 test.The measurement data were expressed as mean±standard deviation. Statistic analysis of multiple groups were performed using One-way ANOVA. The analysis of correlation between OX-LDL and SP-S was analyzed by Pearson linear correlation. The analysis of risk factors for carotid IMT was performed using multiple stepwise linear regression. The analysis of risk factors for carotid atherosclerosis adopt the two classification non-conditional Logistic regression. A value of P <0.05 was considered as statistical significant.Results1. The levels of SBP, DBP, TG, FBG, WC, BMI, UA in A group and B group were significantly higher than the control group (P <0.01) , and the levels of HDL-C were significantly lower (P <0.01) . The levels of SBP, FBG, WC in B group were significantly higher than A group (P <0.01), and the levels of HDL-C were significantly lower (P <0.01).2. Compared with the control group , OX-LDL levels , SP-S levels, IMT, and the incidence of plaque incidence in A group(35.21±7.99 ug/L,71.12±13.62ug/L,0.86±0.12mm,38.1%,respectively)were significantly higher (P <0.01). Compared with the A group , OX-LDL levels, SP-S levels, IMT, the incidence of plaque incidence in B group(54.87±8.71ug/L,90.59±19.94ug/L,1.02±0.11mm,47.0%, respectively) was significantly higher (P <0.01).3. The analysis of correlation show OX-LDL levels has a significant positive correlation with SP-S levels(r=0.72,P<0.01).4. The multiple stepwise linear regression analysis of risk factors for carotid IMT showed OX-LDL, SP-S, SBP, UA, TG, FBG independently associated with IMT, while OX-LDL contribute lagerly to the IMT.5. To the carotid atherosclerosis as dependent variable, SBP, FBG, SP-S,OX-LDL , HDL-C, UA, as independent variables, two classification non-conditional Logistic regression analysis results shown SBP, OX-LDL, FBG, UA are independent risk factors for carotid atherosclerosis of MS patients, which the OR value(95%CI) were 4.531(1.9253-10.6609), 1.1175(1.0596-1.1786), 1.0564(1.0123-1.1024), 1.1206(1.0563-1.2130), respectively, Confidence interval greater than 1.Conclusion:1. With the increasing of the number of MS diagnostic criteria, the levels of serum OX-LDL and SP-S ,the thickness of carotid artery IMT, the incident plaque are significantly increased.2. The serum OX-LDL levels are significant positive correlation with the serum SP-S levels .3. The levels of Serum OX-LDL is one of independent risk factors of carotid atherosclerosis with MS, and could evaluates the disease of carotid atherosclerosis with metabolic syndrome.
