ObjectiveChronic heart failure (CHF) is the main fatality reason in cardiovascular patients.The survival rate of 5 years was similar to menace neoplasm.The main genesis mechanism of CHF is ventricular remodeling,the reactive of neuro-endocrine system and the elevated level of inflammatory factor,such as tumor necrosis factor -α(TNF-α),interleukin-18(IL-18),et al.In this study, we established case group by the patients with CHF and control group by health population.TNF-α,IL-18,left ventricular ejective fraction were detected,to compare the difference in two groups.The case group was divided into conventional therapy sub-group and statins sub-group by different medication.28 days later,repeat to measure the serum level of TNF-α, IL-18, left ventricular ejective fraction in different group subject. The analysis was used to explore the effect of fluvastatin to inflammatory factors in patients with CHF.Methods1,56 patients were taken as CHF group, basic disease included coronary artery disease(CAD),hypertensive heart disease,this group is not including people who have acute and chronic infection disease,tumor,diabetes mellitus,collagenosis,renal disfunction,thyroid disease,myocardial infarction (in half a year),peripheral vascular disease and people who took hypolipidemic 4 weeks before in hospital.2,32 healthy people were taken as normal group, who had no heart,brain,kidney,lung or endocrine disease and it was normal in blood urine convention,blood glucose,serum lipid,liver renal function,ECG,X-Ray chest.3,Writer took face to face interview personally and filled questionnaire. Learned the information about CHF from doctor in charge and progress note. 4,ELISA was taken to test the level of TNF-α,IL-18 in every group.The hypersound was used to test LVEF.5,Case group was divided into conventional sub-group and statins sub-group by different medical therapy.The patients in conventional sub-group were given ACEI, ARB,βreceptor blocker, diuretic agent, et al. the patients in statins sub-group were given additional fluvstatin on the base of conventional therapy.6,TNF-α,IL-18 and LVEF of the two subset were taken after 28 days and compared their difference again. Data arrangementAll data were put into the computer with the software of Excel and analyzed with the SPSS16.0 software.The measurement data were shown by (mean±SD).Independent sample T check was used in in-group comparison.The comparison of sample mean in before and after treatment took paired/matched T check; the size of check wasα=0.05.Results1 The comparation of characteristics between case group and control group. There were no significant difference at age, gender, routine blood assay, hepatic function,renal function,myocardial enzyme and blood lipid between two groups.2 The comparation of serum level of inflammatory factors and LVEF in two groups.The serum level of TNF-αwas higher in case group than in control group (464.65±31.61pg/ml vs 56.24±5.76pg/ml,P<0.05).The serum level of IL-18 was higher in case group than in control group (40.81±5.50pg/ml vs 32.64±4.47pg/ml, P<0.05). LVEF was lower in case group than in control group (33.83±3.18% vs 56.24±5.76%, P<0.05).3 The comparation of characteristics between conventional treatment sub-group and statins sub-group.There were no significant difference at common characteristics and the serum level of TNF-α, IL-8, LVEF. 4 The comparation of the serum level of case group before and after treatment. After provided treatment for 4 weeks,the serum level of TNF-α was lower in statins sub-group than which in conventional treatment sub-group (299.86±31.70pg/ml vs 389.61±29.06pg/ml,P<0.05).The serum level of IL-8 was lower in statins sub-group than which in conventional treatment sub-group (33.23±4.31pg/ml vs 35.55±4.13 pg/ml,P<0.05). LVEF was higher in statins sub-group than which in conventional treatment sub-group (40.78±2.70% vs 37.60±2.42%, P<0.05).Conclusions1 The over express of inflammatory factors promote the remodeling of myocardial muscle, injury of myocardial muscle and deterioration of heart function in CHF patients.2 The Fluvastatin used in a short time can reduce the TNF-αand IL-18 further and enhance the LVEF, which is benefit to the CHF patients (including CAD patients & non-CAD patients).The mechanism of action may be in relation with situation that statins can inhibit the inflammatory reaction of CHF patients.
