Clinical Observation About Wenshenfang Treat With Subclinical Hypothyroidism

[Objective]The objective,through clinical observation of subclinical hypothyroidism in patients with early intervention, contrasted of Chinese medicine, Western medicine, combining Western and Chinese medicine treatment of the three methods, and evaluated the clinical effect of WenshenFang of subclinical hypothyroidism' s treatment[Information]Traditional Chinese Medicine Hospital in Hubei Province received and cured 75 cases the patients they were subclinical hypothyroidism in from November 2009 to February 2010. The diagnostic criteria of subclinical hypothyroidism agreed with the ((Diagnosis and treatment of thyroid diseases in China Guide》by Endocrinology Branch of the Chinese Medical Association in 2007. There were elevated TSH and normal FT3,FT4 in serum. The patients did not contain other diseases by elevating TSH.2. The diagnostic criteria of Consumptive disease agreed with the《TCM disease diagnosis concentration-response standards》in 1995. Patients'performance accorded with consumptive disease. Patients, belonging to the kidney Yang deficiency, showed lazy, pale, cold and ache of extremities, thinning hair, lossing of appetite, abdominal distension, menstruation or amenorrhoea of women, decreaseing sexual appetite of men,pale and white tongue, sink or late.pulse etc.3. Age ranged for 18 to 60 years. The course was no more than one year. The patients did not have serious disease of heart, liver, kidney disease.4. Willing to accecp clinical trials and to forecast better compliance.[Method]To Select meet diagnostic criteria for 75 cases of patients of subclinical hypothyroidism, randomly divided into three groups: chinese medicine treatment group, western medicine treatment group, combine western treatment group. Each group contained 25 cases. Chinese medicine treatment group uesed TCM. Western medicine group was given L-T4. Combine western treatment group was given TCM and L-T4. The duration of a course of treatment was one month. The treatment of three groups of patients continued three months. The therapeutic effect was assessed after three courses of treatment. Safety indicators included routine blood, routine urine, liver function, renal and ECG before treatment. Clinical assessment were thyroid function(FT3, FT4,TSH), lipid(TC,TG, LDL-C, HDL-C), cardiac function (LVEF, LVFS, LVPW) and symptoms.[Results]1. Comparison of integrated effects of three groups of syndromes, takeing patients experienced clinical symptoms of press powerfully, effective, invalid investigation, displayed combine treatment group with significant efficiency and total effective (P<0.05) better than others. Western medicine group had total effectiveness better than Chinese medicine group (P<0.05)2. Three kinds of treatments can reduce TSH levels. There was significant difference between each group after treatment (P< 0.05). Combine medicine group was significantly effect better than other groups (P< 0.05)3. In same group the change was significant comparative differences after the treatment (P<0.05). Three kinds of treatments can reduce TC and LDL-C levels. But the effect of improving TG and HDL-C levels was not obvious (P> 0.05). In different group the change was significant comparative differences after the treatment (P< 0.05).Combine medicine group was better than other groups in reducing TC and LDL-C levels (P<0.05).Secondly, western medicine group was superior to traditional chinese medicine group in reducing TC and LDL-C levels (P<0.05)4. Left ventricular systolic function increased mildiy in same group after the treatment. But the change was not significant differences intraclass comparison (P> 0.05). It was not statistical significant in comparison among groups (P>0.05).[conclusion]The effect,that Wenshenfang combined with western medicine,was better than pure chinese medicine or routine medicine. Integrative medicine group can regulate effectively the thyroid hormone and lipid levels. At the same time, it can alleviate effectively clinical symptoms. Early treatment of subclinical hypothyroidism had positive significances. there were ameliorating thyroid hormone and Lipid levels, reducing risk factors for heart. The total effective rate and the level of thyroid hormone in the integrative medicine group were significantly higher than the other groups. Wenshenfang combine with western medicine had clinical extension.
