BackgroundPeriodontitis is an infectious peridentalt disease which is caused by dental plaque biofilm's destroy of supporting tissues of gum, pericementum, alve- olar bone and cementum by forming periodontal pocket, inflammation, con -tinual periodontal loss of attachment and alveolar resorption. Researches on causes of Periodontitis and its mechanism have never ceased.Haffajee and his colleagues put forward the theory of Environment-Plague in 1991, which stresses the crucial role of subgingiva microorganism envi- ronment in the development of the disease. Presently, there are proof indi- cating that the theory is applicable to chronic periodontitis.Marshall and Warren separated Hp from biopsy sample from gastric mucosa of chronic ongoing gastritis in 1983 which because a hot issue in international circle of digestive disease studies. Later various evidences show that Hp is the pathogen of digestive diseases. In 1990, Maimudar was reported to have separated Hp from bacterial plaque in Hp infectious gastritis patients and later, researchers emphasized more on Oral Hp studies. In recent years, researchers stress more on the relevance study between periodontitis and diseases on other body parts. Concerning whether there is relevance between Hp and periodontitis, periodontitis and digestive tract diseases, there are still controversies.Periodontal disease morbidity in 80% ~ 90% in China, is the main cause of adult teeth loss. Sufficient evidence that the plaque mic- robes are tumor-initiating factor, gum disease defense responses is host of periodontal disease occurrence, development and pro- gnosis of a necessary condition. A lot of research shows that some systemic disease, systemic factors and behavior can affect the health of periodontal tissue; Periodontal infections is also certain systemic disease risk factors. With the accumulation of the rele- vant evidence, especially the concept of periodontal medical,pe- riodontal disease and whole body healthy correlation become hot- spotObjectiveCheck the Hp infection in the mouth of periodontitis patients, discuss the relevance and relevance mechanism between CP and digestive tract disea- ses by making clinical investigation of oral Hp, CP and digestive tract di- sease relevance in order to provide valuable information of determining the causes of CP and lowering the risky factors causing the recrudescence of digestive tract diseases.The research mainly includes the following items:①making clinical investigation of relevance between CP and digestive tract diseases on the basis of similarities of oral mucosa and gastric mucosa and Hp cultivation in oral bacterial plaque.②probing into relationships of oralHp and CP and digestive tract diseases③discussing the eradication effect of oral Hp of basic periodontium cure by rapid urease test in the Hp positiveness of the CP patients.Methods:This research is divided into three parts1. Periodontitis and digestive disorders relevance of clinical investigation and analysisThe experiment group includes 562 patients of periodontal and oral mucosa diseases in Tangshan Xiehe Hospital from January to October in 2010, with 297 males and 265 females, aged from 11 to 68 (average 36.7). All these patients suffer from chronic periodontitis. The normal control group inclu- des 66 patients of non-periodontitis in periodontal and oral mucosa disea- ses in Tangshan Xiehe Hospital, with 35 males and 31 females of average age of 36.7. Investigation tables are designed to launch the Epidemiological investigation. A clear diagnosis was given after regular checks and inspect- tions. All patients were given standard question about their medical history, with their basic information of gender, age and contact method carefully re- corded. A detailed record was made about the patients' periodontium index, seriousness of pathological changes, situation of the oral health and living habit as well as the parodontopathy medical history of lineal relatives of patients, symptoms of digestive tract diseases. A careful and comprehensive information collection can ensure the relevance between periodontitis and digestive tract diseases from the perspective of clinical epidemiological in- vesttigation.2. Chronic periodontitis and oral Hp relevance of clinical analysis.The experiment group and normal control group same to first survey. Inves- tigation tables are designed to launch the Epidemiological investigation. A clear diagnosis was given after regular checks and inspec- tions. A detailed record was made about the patients' perio- dontium index, serious- ness of pathological changes, situation of the oral health and living habit as well as the parodontopathy medical history of lineal relatives of patients, symptoms of digestive tract diseases. the oral Hp tested by rapid urease testing and clear False Positive by rinsing the mouth with Low concentra- tion vinegar(pH=6.4).A careful and comprehen- sive information collection can ensure the relevance between the oral Hp and periodontitis, oral Hp and digestive tract diseases from the perspective of clinical epidemiological investigation.3. Analysis of oral Hp positie therapeutic effect . Analysis of effects of basic periodontium cure to Hp-positive patients with digestive tract diseases: 59 patients were chosen from the first group recei- ving epidemiological investigation according to standards of case selection, with 30 males and 29 females of average age 38.6. Them regroup the pa- tients randomly, with experiment group of 15 males and 15 females, age averaged 37.3; the control group includes 15 males and 14 females, age averaged 39.5. The basic periodontium cure was offered to the experiment group (with a united use of scaling, curettage and injection of minoeyeline hydrochloride into the periodontal pocket. The normal group is not given the injection of minoeyeline hydrochloride into the periodontal pocket. Contrast the Hp clean effects of the two methods.Results1.Among the 66 periodontium-healthy patients, 4 is with digestive tract di- sease, the ratio is 6.1%; Among the 306 slight chronic periodontitis patien- ts, 31 are with digestive tract disease, the ratio is 10.1%; Among the 77 mo- derate chronic periodontitis patients, 15 are with digestive tract disease, the ratio is 19.5%; Among the 113 heavy chronic periodontitis patients, 35 are with digestive tract disease, the ratio is 31.09%. A statistical difference was found afterχ~2 check of digestive tract disease rate of CP patients and periodontium-healthy patients, P<0.05 which shows that the digestive tract disease rate of CP patients is higher than that of periodontium-healthy pa- tients.2. Among the 470 patients of no digestive tract disease, 403 are of positive oral Hp, the ratio is 85.70%; Among the 92 patients of digestive tract disea- se, 87 are of positive oral Hp, the ratio is 94.5%; A statistical difference was found afterχ~2 check of oral Hp infection rate of digestive tract disease patients and patients of no digestive tract disease, P<0.05 which shows that the oral Hp infection rate of digestive tract disease patients than that of pa- tients of no digestive tract disease.Among the 306 patients of slight periodontitis, there are 264 of positive oral Hp, with a ratio of 86.3%; Among the 77 patients of moderate perio- dontitis, there are 68 of positive oral Hp, with a ratio of 88.3%; Among the 113 patients of heavy periodontitis, there are 113 of positive oral Hp, with a ratio of 93.8%; Among the 66 patients of no periodontitis, there are 41 of positive oral Hp, with a ratio of 62.1%; a statistical difference was found afterχ~2 check of oral Hp infection rate of CP patients and periodontium- healthy patients, P<0.05 which shows that the oral Hp infection rate of CP patients than that of periodontium-healthy patients and that the heavier the periodontitis is, the higher rate of oral Hp infection rate is.3. Among the experiment group of 30 patients who received basic perio- dontium cure and medicine for three months, 24 are of positive oral Hp, the ratio is 80.0%; In the control group of 29 patients , 24 are of positive oral Hp, the ratio is 82.8%. A statistical difference was not found afterχ~2 ch- eck , P>0.05 which shows that the oral Hp clean effect of basic periodon- tium cure is the same as that of patients of medical cure.Among the experiment group of 30 patients who received basic periodontium cure and medicine for three months, 24 are of positive oral Hp, the ratio is 80.0%; In the control group of 29 patients , 24 are of posi- tive oral Hp, the ratio is 82.8%. A statistical difference was not found afterχ~2 check , P>0.05 which shows that the oral Hp clean effect of basic pe- riodontium cure is the same as that of patients of medical cure.One week later, among the experiment group of 30 patients who received basic periodontium cure and medicine, 11 are of positive oral Hp, the ratio is 37.9%; In the control group of 29 patients , 16 are of positive oral Hp, the ratio is 55.2%. A statistical difference was not found afterχ~2 check , P>0.05 which shows that the oral Hp clean effect of basic periodontium cure of one month is the same as that of patients of medical cure. Conclusion1. The digestive tract disease occurrence rate of CP patients and non-CP patients shows a remarkable difference (P<0.05) in the research. The diges- tive tract disease occurrence rate of CP patients is obviously higher than of non-CP patients2. There is a statistical difference in the oral Hp infection rate of patients with digestive tract disease and patients of no digestive tract disease, P<0.05. The oral Hp infection rate of patients with digestive tract disease is higher than that of no digestive tract disease.3. The Hp infection rate of CP patients is higher than that of healthy people, P<0.05. The heavier the periodontitis is, the higher the Hp infection rate is.4. The experient proves that basic periodontium cure and a joint cure of basic periodontium cure and medicine can both eradicate oral Hp. After treatment for one week, the effect of a joint cure of basic periodontium cure and medicineperiodontium cure is better than that of single basic periodon- tium cure. But in contrast after one month and three months, there is no di- fferrence between the effects. Basic periodontium cure is an effective method in eradicating oral Hp.
