ObjectiveEndometrial carcinoma is one of the three common malignant tumors in the female reproductive system, in recent years,the incidence trend is rising. At present,the pathogenesis of endometrial cancer is not very clear, so exploreing the endometrial cancer's mechanism of invasion and metastasis can be to provide evidence for the prognosis of endometrialcancer patients.Osteopontin(OPN) is a kind of secretion type calcium binding glycoprotein which has a variety of functions, and it is an important component of extracellular matrix , have a wide variety of sources, they are expressed in many tissues, sometimes it have a trace expressed in the normal body fluids. In the process of tumor development, OPN can be binding to adhesion molecules CD44, and involved in cell adhesion, migration and proliferation, inhibited apoptosis, thus mediated the growth and metastasis of tumor, and related with tumor dissemination and immune evasion[1]. Many studies confirmed that, in a variety of humantumors, OPN has a high expression level in tumor tissue or plasma, and it is significantly higher than normal tissue, and have a clear correlation with malignant stages and parts[2], and have a close relationship with the prognosis of advanced tumors.Extracellular matrix (estracellular matrix, ECM) is an important physical barrier, while the degradation of ECM is an important way with the invasion and metastasis of tumors.Matrix metalloproteinases (MMPs) is a kind of proteolytic enzymes, which is dependence in Zn2+ and Ca2+ and can biodegrad extracellular matrix (ECM). MMPs plays a critical role in the degradation of extracellular matrix, And also involved in the formation of the primary tumor, tumor angiogenesis, inducted immune tolerance of tumor cells. MMP-2 (Matrix matallproteinase-2) are gelatinases, that is an important member of the family of MMPs, and it has a function in the degradation of vascular basement membrane and remodelition of extracellular matrix[3].it can participate in and accelerate the tumor's invasion and metastasis[4].This study maked use of immunohistochemistry detecting the expression of OPN and MMP - 2 in normal endometrium, atypical end- ometrial and endometrial carcinoma, thus explores the relationship with both and the endometrial cancer's occurrence, development and infiltration, transfer, and provides the theory basis for endometrial cancer prognosis assessment and drug development.Methods and experimental objectExperimental object is selected 68 examples endometrial cancer pat- ients who are treated in tangshan kailuan hospital department from January 2003 to May 2010, the cases and operation recorded data is comp- lete. All biopsies are staining by HE dyeing, carries on diagnosis and correction group according to the clinical and pathological features. Patients were not taken preoperative chemotherapy, radiotherapy and hormone therapy.We selected the same period 28 examples endometrial atypical hyperplasia patients as control group, that was diagnosticed according to pathological diagnosis, who was carried out diagnostic curettage. During the same period, we selected 28 examples hyperplasia period and secretory period's normal endometrial as control group,which came from the uterine fibroids or uterine prolapse patients that was misdignosed in gynecological.MethodsAfter the fixation specimens were made wax block, sliced, then we carried out immunohistochemical staining, image acquisition and quan- titative analysis used mike audi digital medical image analysis system(Mo- tic. J Med), the positive dyeing signal of the slice were analyzed throu- gh average light density. We detected the expression of OPN and MMP-2 in detection normal endometrium organization, atypical endometrial orga- nization and endometrial carcinoma tissues with quantitative method. We used SPSS16.0 statistics software sorting and statistical analysising income data.The results1 In the normal endometrium group, the atypical endometrial group and the endometrial cancer group, the expression differences of OPN are statistically significant (F = 33.893,P < 0.01). In the three groups, endome- trial cancer groups are higher than normal endometrium group and atypical group (P < 0.01), endometrial atypical groups are higher than normal endometrium groups (P < 0.01).2 In the endometrial cancer groups, with surgery - pathological instalments heighten (F = 5.106,P < 0.01), muscular layer infiltration of deepening (F = 10.943,P < 0.01), histologic classification heighten (F= 10.536,P < 0.01), and the increase of lymph node metastases (t = - 3.900,P < 0.01), the expression of OPN is enhancement, while in different pathological types, the expression was not statistically significant (F = 0.855,P = 0.469).3 In the normal endometrium group, the atypical endometrial group and the endometrial cancer group, the expression differences of MMP-2 are statistically significant (F = 29.646,P < 0.01). In the three groups, end- ometrial cancer groups are higher than normal endometrium groups and atypical endometrium groups (P < 0.05), the differences between endom- etrial atypical groups and normal endometrium groups was not statistically significant (P = 0.118).4 In the endometrial cancer groups, with surgery-pathological instalments heighten (F = 5.528,P < 0.01), muscular layer infiltration of deepening (F = 17.316,P < 0.01), histologic classification heighten (F= 11.032,P < 0.01), and the increase of lymph node metastases (t = - 4.454,P < 0.01), the exp- ression of OPN is enhancement, while in different pathological types , the expression was not statistically significant (F = 1.400,P = 0.251).5 In the normal endometrium groups, atypical endometrial groups and endometrial cancer groups, the difference expression of OPN and MMP-2 was not statistically significant (t = - 1.282, P = 0.205) (t = 0.942, P = 0.350) (t = 0.429, P = 0.669).6 In normal endometrium group, atypical group and endometrial cancer group, the express of MMP-2 and OPN have the positive correlation(r = 0.649, P < 0.01) and (r = 0.651, P < 0.01) (r = 0.634, P < 0.01).Conclusion1 The espressions of OPN in the endometrial carcinoma groups are higher than them in the normal endometrium groups and atypical endometrial groups.2 In the endometrial cancer groups, the expressions of OPN have a closely relation with surgery-pathological staging, muscular layer infiltration, histologic classification, with or no lymph node metastasis , while has nothing to do with pathological type.3 The expressions of MMP-2 in the endometrial carcinoma groups are higher than them in the normal endometrium groups and atypical endometrial groups.4 In the endometrial cancer groups, the expressions of MMP-2 have a closely relation with surgery-pathological staging, muscular layer infiltration, histologic classification, with or no lymph node metastasis , while has nothing to do with pathological type.5 In the normal endometrium groups, the atypical groups and the endomet- rial cancer groups, the expressions of OPN and MMP-2 are creaseing at the same time.
