Clinical Analysis With Rigid Internal Fixation In 162 Cases Mandibular Fractures

Objective: To explore the application and effect of rigid internal fixation on mandibular fractures, and thus provide the theoretical foundation and summary some feelings for the rigid internal fixation on mandibular fractures with pure titanium.Methods: 162 patients with mandibular fractures treated by rigid internal fixation with pure titanium plates were chosen from the patients treated in the Department of Oral and Maxillofacial surgery, Dalian Medical University Subsidiary Hospital from June 2005 to December 2010. The group of patients were analyzed according to age, gender, cause of injury, position of fracture, accompanying systemic injuries, operative approach, setting of titanium plate,the tooth of fracture line,post operative eating, off plate, occluding relation after surgery, complication after surgery and so on. Then, the data were displayed by statistic chart. We analyzed the data and then discussed as follows.Results: Among this 162 patients, most of male,most hurtby accident, most happened young.The 231 fractures happened in the 162 patients. Among this 231 fractures adopt RIF treatment in 216, most was symphysis fracture,which was the most popular fracture, next was mandibular condylar fractures. Among this group of patients, internal approach was used most extensively;Every fracture line rigid with two plates. Total infection 10, none of the process mode of tooth in fracture line business. Most op-infection happened mandibie angle fracture and comminuted fracture. Above two fracture and commimuted fracture easy happen occlusal interference after opration. No permanently trismus appear. Mandibular alveolar nerve injury always happened before operation. 162 patients after RIF treatment are healed well. According to anatomical reduction, rigid fixed, noninvasive surgery, early function, four principles. reduce the complications , shorten treatment, early recovery jaws function, early chew food. Conclusion: The rigid internal fixation of pure titanium plates applying to mandibular fractures is feel better, chew food, clinic care, occluding relation and recovery of open mouth extent after surgery, it can use any of the part of mandibular fracture. It can get more stable and favorable fracture healing that use double plates. It is better to treat occlusal interference after operation use elastic bandage which used to link skull and jaw. If there is a wound can be use, use it. Except that condylar fracture, complex mandibular angle and high mandibular ramus fracture, we take buccal small incision combined with small incision use wear buccal instrument playing nail fixed outside, we always use the internal approach, avoid postoperative infection.
